As part of the employment process,
(the Company) who is a vendor or service
provider and its client (the Sponsor) may obtain information about you for employment purposes from a third party consumer
reporting agency. Thus, you may be the subject of a consumer report and/or an investigative consumer report which may
include information about your character, general reputation, personal characteristics, and/or mode of living, and which can
involve personal interviews with sources such as your neighbors, friends, or associates. These reports may contain
information regarding your credit history, criminal history, social security verification, motor vehicle records (driving records),
verification of your education or employment history, or other background checks. Credit history will only be requested where
such information is related to the duties and responsibilities of the position for which you are applying. You have the right,
upon written request made within a reasonable time after receipt of this notice, to request disclosure of the nature and scope
of any investigative consumer report and a copy of any report about you. Please be advised that the nature and scope of the
most common form of investigative consumer report obtained with regard to applicants for employment is an investigation into
your education and/or employment history conducted by First Advantage, P.O. Box 105108, Atlanta, GA
30348-5108,1-800-845-6004, www.fadv.com/risk/solutions. The scope of this notice and authorization is all-encompassing,
however, allowing the Company and Sponsor to obtain from any outside organization all manner of consumer reports and
investigative consumer reports now and throughout the course of your employment to the extent permitted by law. As a
result, you should carefully consider whether to exercise your right to request disclosure of the nature and scope of any
investigative consumer report.
New York and Maine applicants or employees only: You have the right to inspect and receive a copy of any
investigative consumer report requested by the Company by contacting the consumer reporting agency identified above
directly. You may also contact the Company to request the name, address and telephone number of the nearest unit of the
consumer reporting agency designated to handle inquiries, which the Company shall provide within 5 days.

A Summary of Your Rights Under the Fair Credit Reporting Act
The federal Fair Credit Reporting Act (FCRA) promotes the accuracy, fairness, and privacy of information
in the files of consumer reporting agencies. There are many types of consumer reporting agencies,
including credit bureaus and specialty agencies (such as agencies that sell information about check writing
histories, medical records, and rental history records). Here is a summary of your major rights under the
FCRA. For more information, including information about additional rights, go to

You must be told if information in your file has been used against you. Anyone who uses a credit
report or another type of consumer report to deny your application for credit, insurance, or employment

You have the right to know what is in your file. You may request and obtain all the information
about you in the files of a consumer reporting agency (your file disclosure). You will be required to

You have the right to dispute incomplete or inaccurate information. If you identify information in

Consumer reporting agencies must correct or delete inaccurate, incomplete, or unverifiable


information. Inaccurate, incomplete or unverifiable information must be removed or corrected, usually

Consumer reporting agencies may not report outdated negative information. In most cases, a
consumer reporting agency may not report negative information that is more than seven years old, or
bankruptcies that are more than 10 years old.

Access to your file is limited. A consumer reporting agency may provide information about you only

You must give your consent for reports to be provided to employers. A consumer reporting agency
may not give out information about you to your employer, or a potential employer, without your
written consent given to the employer. Written consent generally is not required in the trucking

You may limit "prescreened" offers of credit and insurance you get based on information in
your credit report. Unsolicited "prescreened" offers for credit and insurance must include a toll-free

You may seek damages from violators. If a consumer reporting agency, or, in some cases, a user of

CODE OF CONDUCT

[Advent Global Solutions] is strongly committed to providing quality service to its customers. In furtherance of
this commitment, each Contingent Staff worker is required to be productive, reliable and trustworthy.
To assist the Contingent Staff in meeting these requirements, [Advent Global Solutions] provides the following
list of conduct which is prohibited and which will subject an individual to disciplinary action, up to and
including discharge. This is not an exhaustive list of every form of conduct, which may result in disciplinary
actions (list is subject to modification):
1.

Excessive absenteeism, tardiness or leaving early.

2.

Failing to follow prescribed call-in procedures in event of absence/tardiness. No call, no show for work
is of particular concern.

3.

Distributing, possession or using illegal drugs and/or alcohol while on duty or in the workplace.
Reporting to work or performing duties while under the influence of drugs and/or alcohol.

4.

Failing to follow all applicable safety rules and guidelines.

5.

Theft, damage, destruction or misuse of company or customer property.

6.

Falsification or misrepresentation on any company document, including but not limited to, application
for employment and time records.

7.

Sexual harassment or any other form of harassment of co-workers or other individuals at the work site
or while engaged in company business.

8.

Rudeness to co-workers, customer personnel, the public, or others at the work site or while engaged in
company business.

9.

Hanging up on customers or manipulating the telephone system to avoid taking calls. (Calls are
subject to monitoring.)

10.

Possessing firearms, explosives or other weapons while at work site or engaged in company business.
Individuals with a concealed weapons permit are subject to this prohibition.

11.

Refusing to work overtime when required.

12.

Sleeping on the job.

13.

Making unauthorized personal telephone calls while at the work site or engaged in company business.
Using company or customer telephones to make unauthorized long-distance telephone calls.

ACKNOWLEDGMENT OF STATUS AS
[Advent Global Solutions] EMPLOYEE AND CONFIDENTIALITY AGREEMENT
I understand and acknowledge that I am being hired as an employee of [Advent Global Solutions] or its
subcontractor to perform work on UPS or non-UPS property. In consideration for my employment by [Advent
Global Solutions] or its subcontractor, I knowingly and voluntarily agree to the following:

I will be employed solely by [Advent Global Solutions] or its subcontractor and will be subject
to [Advent Global Solutions]'s or its subcontractors employment policies and may participate in
any [Advent Global Solutions] or subcontractor benefit plans for which I am eligible.

I am and will remain solely an employee of [Advent Global Solutions] or its subcontractor at all
times during the performance of this work.

I will not be employed by, subject to the employment policies of, nor be eligible for or
participate in any employment benefits or benefit plans of UPS or any of its affiliates or
customers and I will not assert a claim of employment against UPS, its affiliates, or customers
nor claim any entitlement to participation in their benefit programs. If, however, I am deemed
to be eligible for participation in such benefits or plans, I hereby waive and release any such
rights.

I will not in any way seek or ask about employment by UPS or its affiliates while I am
employed by [Advent Global Solutions] or its subcontractor.

I will maintain in strict confidence and use solely as necessary to performance my


responsibilities while working on the UPS account, all information received or accessed while
working on the UPS account that is marked as UPS confidential information, is the subject of
reasonable efforts by UPS or its affiliates to maintain its confidentiality and is not generally
disclosed to persons not employed or otherwise engaged by UPS, or is of a nature, due to the
information or the circumstances surrounding its disclosure or acquisition, that I reasonably
should have known to be confidential (Confidential Information). I will protect and limit use
of the Confidential Information during the entire time of this work on the UPS account and for
five years afterwards except that such restrictions regarding Confidential Information will apply
in the case of trade secrets, so long as such Confidential Information remains a trade secret or
for such five-year period, whichever is longer.

UPS shall own all right, title and interest in and to all materials developed by me (whether
independently or jointly with UPS), including, without limitation, all inventions, software
(including any object or source code related thereto), works of authorship, whether patentable or
unpatentable, which are first conceived or made or first actually or constructively reduced to
practice while working on the UPS account, including all intellectual property rights therein
(Work Product). I expressly acknowledge that all aspects of the Work Product and all work
in process in connection therewith are to be considered works made for hire within the
meaning of the United States Copyright Act of 1976, as amended, and that UPS is to be the
author within the meaning of such Act. All such copyrightable Work Product in whatever
medium fixed or embodied, shall be owned exclusively by UPS at its creation, and I hereby
expressly disclaim any interest in any of them. In the event (and to the extent) that any Work
Product, or part or element thereof is found as a matter of law not to be a work made for hire
within the meaning of the Act, I hereby irrevocably convey and assign (and in the case of Work

Product not yet developed, hereby covenant upon their development to irrevocably convey and
assign) to UPS the sole and exclusive right, title and interest in and to all such Work Product,
without further consideration,

In the event that my employment is terminated or I resign, I will not remove, download, copy,
or transfer any software, material, equipment resources or Confidential Information from any
UPS facility. Additionally, I will return immediately all information, documents and other
property pertaining to my performance of services on the UPS account.

COMPUTER USE GUIDELINES
I. DATA SECURITY
GENERAL It is the responsibility of every user to have a thorough understanding of the Data Security
Guidelines. The guidelines are set to protect UPS resources from damage or loss. Adherence to these
guidelines is everyones responsibility.
DATA PROTECTIONS All information on UPS computers, systems, software and networks, and all
Confidential Information, including technology and data, stored on or accessible through utilization of such
systems, software and networks (collectively, UPS Systems) should be adequately protected from accidental
or purposeful disclosure, damage or destruction. It is the responsibility of the user to ensure the protection of all
programs and data in the users care whether on the PC or other external media such as floppy diskettes. In the
event users have access to the UPS Systems in performing services for UPS, users shall adhere to all
requirements that UPS provides for access to or use of the UPS System, including, without limitation, training
requirements, password control measures and other access control measures, physical security requirements, and
virus and other security software installation and maintenance procedures. Users should access, make
maintenance changes to, or perform services for UPS only with respect to the Confidential Information as
required by their job responsibilities for UPS.
ENCRYPTION - SSL with the highest encryption available must be used when transmitting and/or
communicating Confidential Information across the Internet. Any encryption products must be at least
FIPS140-2 certified. Passwords stored in databases must be one-way hashed.
SECURITY ADMINISTRATION Vendor management personnel (Vendor Security Administrators) must
retain sole responsibility for granting access to the UPS Systems and/or Confidential Information for all Vendor
employees and users. Vendor Security Administrators must document all procedures for user ID requests,
transaction authorization, and UPS System use. Vendor Security Administrators must review all violation and/or
monitoring reports and take action as necessary to prevent unauthorized access and use of the UPS Systems
and/or Confidential Information.
WORKSTATION CONTROLS - All workstations which allow access to Confidential Information shall be
controlled. All software used on any workstation must be properly licensed and used in accordance with the
applicable license agreement. All such workstations must be:
(a) located in a physically segregated work area.
(b) positioned to face away from any common areas of the facility such as windows.
(c) equipped with appropriate access control, including password protected screen savers,
and/or time-out after 10 minutes or less of non-use.
(d) configured with current anti-virus software, and provide a mechanism to ensure that the
anti-virus software is kept current.
(e) configured such that they do not enable or facilitate the transfer of Confidential Information
by any portable storage medium (e.g., floppy drives, writeable CD-ROMS, Zip Drives, USB
memory devices, etc.), by email, or by other means such that an employee or agent could
distribute Confidential Information without permission or consent by UPS.

ACTIVITY LOG - Users must log all activities in regards to accessing UPS Systems, including inquiries. This
audit data must be retained for one year.
KEEP MASTER DISKS SECURE Always use the original master diskette when loading software to a system.
When using the master, make sure it is write-protected. On 5.25 inch diskettes, cover the write protect notch
with a silver or black coated write-protect sticker. On 3.5 inch diskettes, slide the plastic square in the upper
right-hand corner so that one may see through the hole. All copyrighted software must be maintained by or
accessible to the UPS Technical Support department.
MAINTAIN A REGULAR BACKUP SCHEDULE Make sure backups of the hard disk are done on a regular
basis. This ensures there is a good copy of the programs and data if infection occurs. All Confidential
Information stored in Vendor repositories or otherwise backed up must be encrypted prior to backup.
COMPUTER VIRUS A virus is an unwarned program that usually enters the computer through some type of
data transfer. A virus can take many forms. Some viruses are simple and easily detected; others are very
sophisticated and may not cause damage until activated months after initial infection. Once a destructive virus
invades a computer, it may be impossible to dislodge. The following guidelines are to be followed by all users
to prevent computer virus infection:
1.

Under NO circumstances are diskettes or any other software media (tapes, CD-ROM disks,
portable drives, etc.) to be brought into the UPS facility or leave a UPS facility without approval
by the local System Manager.

2.

Under NO circumstances are personally owned computers or software allowed to be brought


into a UPS facility. Conversely, no software or hardware may be removed from the UPS
facility without approval by the local System Manager.

3.

Under NO circumstances is software allowed to be downloaded to a UPS computer or network


from a bulletin board, Web Site, or any other location external to the UPS facility except what
has been approved by the local System Manager. If approval has been granted the file(s) must
be downloaded to a diskette and scanned using the latest version of a UPS approved virus scan
utility. See the local technical support department for information about UPS approved virus
scan software.

4.

If a user suspects that the computer is infected with a computer virus, the user should STOP
USING THE COMPUTER. DO NOT PLACE A DISKETTE INTO THE DISK DRIVE OR
CONNECT TO ANY NETWORK. If the PC is infected, backing up or copying data may
spread the virus further.

ELECTRONIC ORGANIZERS The use of personal electronic organizers is prohibited.

II. ON-LINE SERVICES


The following guidelines are to be followed by all users within a UPS facility when accessing any on-line
services.
Do not access the UPS System via a dial-up connection or otherwise from any place except a UPS-approved
site. All users accessing the UPS System via remote access must utilize Vendor approved authentication
mechanisms.

The downloading of programs or other information from unauthorized bulletin boards is prohibited. If special
circumstances justify downloading from electronic bulletin boards, prior approval must be obtained from the
local System Manager. If files are downloaded, the file(s) must be downloaded to diskette(s) and the diskette(s)
must be scanned using the UPS approved virus scan utility.
Programs and files from unauthorized non-UPS sources must not be loaded or used on UPS computers. Observe
the copyright license guidelines when viewing news information.
Any user within a UPS facility accessing on-line services may not engage in any activity that may result in the
establishment of an obligation to UPS financially, contractually, or otherwise. Examples include (but are not
limited to) procurement of goods and services, shareware, delivery of research material or information, data
collection, etc.
Non-disclosure (Copyrights, Protection of Intellectual Property, and Non-disclosure) UPS is committed to
protecting its intellectual property because of the significant value intellectual property has in terms of UPSs
future and strategic planning. Because the disclosure of proprietary information can occur electronically,
exercise caution when using electronic mail, network news, remote log, and file transfer facilities.
User will not, without UPSs prior written permission, disclose to anyone outside of UPS, or use in other than
UPSs business, any Confidential Information, or any information or material received by UPS in confidence
from third parties. Upon termination of users services for UPS, user will return all property of UPS in his/her
possession, including all Confidential Information.
III. UPS SOFTWARE COPYRIGHT COMPLIANCE
The licensed use of software provided to UPS by software Vendors and publishers is a privilege. It is the
responsibility of all users to observe the UPS Copyright Laws not only to avoid personal liability of the
individual, but also to protect UPSs right to use important business tools on which the company has come to
depend. Violators of the software copyright compliance face fines up to $100,000 for each illegal copy if
punished under civil law. Violators charged under criminal law can be fined up to $250,000 for each violation
and sent to prison for up to five years.
To provide protection, the following rules must be observed at all times:

DUPLICATION OF SOFTWARE IS PROHIBITED FOR ANY PURPOSE OTHER THAN BACKUP


OR ARCHIVING The only personnel authorized to duplicate software for use in any production mode
are Project Implementation personnel in National Information Services. UPS is only authorized by
Vendors to make copies of software in select instances, and Project Implementation is the only area
within UPS permitted to do so. Any unauthorized copying is a crime and exposes the individual and
UPS to civil and criminal liability and revocation of license to use the software.

THE USE OF SOFTWARE OBTAINED THROUGH AN UNAUTHORIZED SOURCE IS PROHIBITED


Examples of such software from unauthorized sources would be any type of downloaded software from bulletin
boards, shareware, freeware, or software from home or other personal contacts. The use of this software
heightens the risk of copyright violation as well as the risk of data destruction by computer virus. All master
diskette, license, and packaging material should b readily accessible for the workstation to which it is assigned.
One UPS utilities disk is licensed for a PC only.
FULL COOPERATION IN THE SOFTWARE AUDIT PROCESS IS REQUIRED OF ALL USERS Software audits are an essential part of out efforts to protect confidential and copyrighted information. These
audits help UPS avoid legal liability. The cooperation and full support of the audit process by all users will

reduce the incidence of software piracy and other unauthorized use and also provide legal evidence of our good
faith efforts to protect the valuable property of those who have entrusted it to our care.
REPORT ANY KNOWN OR SUSPECTED VIOLATIONS TO YOUR SUPERVISOR It is the responsibility
of every user to preserve UPSs right to use third party software by ensuring strict compliance with these rules.
Any unauthorized activities that place UPSs license rights in jeopardy must be promptly reported (anonymously
if you wish) to the appropriate supervisor to avoid increased risk.
NEGOTIATED TERMS - The parties agree that the language terms, conditions and provisions of this
Agreement are the result of negotiations between the parties and that this Agreement shall not be
construed in favor of or against any party by reason of the extent to which any party or its professional
advisors participated in the preparation of this Agreement or based on a partys undertaking of an
obligation under this Agreement.
To preserve copyright protection, any suspected acts of infringement must be immediately reported to the
local Security department.

We have developed Safe Work Methods to help provide a safety culture on your assignment at UPS.
The UPS Safe Work Methods support the fact that no job is so important that you have to risk injury to
get the job done.

Job Setup
Preparation for the workday prevents injuries
-Dressed for job
Cold weather requires layers to absorb the sweat, to protect your skin and to keep your
body warm. In hot and/or humid weather, it is recommended that one wear cotton or breathable
clothing. Clothing should be comfortable and loose but not baggy. Long hair should be
restrained above the shoulders. A sturdy work shoe with slip resistant soles is vital to success
on the job. Substantial work shoes provide needed protection, support, and ultimately prevents
fatigue and injury to the ankles, knees, and lower back.
-Physically ready for the job
Being rested is part of being physically ready to do the job. Adequate rest improves
concentration and makes us more efficient. Stretching helps our bodies wake up and prepare for
activity. Hydration is essentialDrink water early and often.
-Mentally ready for the job
Everyone should arrive early to get organized for the work day. Concentrating and
focusing on the job are vital to completing the job safely and productively.
-Work area ready
Ensure that you have tools/equipment necessary to complete the job safely. Report any
defective tools/equipment immediately. Store tools and equipment properly when finished.

Lifting and Lowering


-Know your Power Zone
The Power Zone is the area within your range of motion that gives you the maximum
strength capability with utmost comfort. When lifting, using the Power Zone means keeping the
object close to the body and from mid-thigh to the arm pits.

-Eliminate or minimize end-range motions


End-range motions occur at extreme, end limits of a body parts movement. For
example, when your elbows move in front of your body while reaching out to lift, you are using
an end-range motion. The farther an object is held away from the body, the greater the strain is
on the back.

The Eight Keys to Lifting and Lowering


1. Get close to the object
Keep the object within your Power Zone, bring the object into your Power Zone by
pulling or sliding it toward your stomach.
2. Position your feet
Your feet should be approximately shoulder width apart, with one foot slightly ahead of
the other so you can lift between your knees. This not only maximizes your ability to keep an
object close, but also gives you the best balance to handle the motion.
3. Bend at the knees
Keep the natural curve of the back: Remember that the muscles in your legs are the
strongest in the body. Bending at the knees maximizes use of these muscles. Also, keeping the
natural curve of the back minimizes forces applied to it.
4. Test the object for weight and shifting contents
Tilt or slide the object to test its weight.
5. Get a firm grip and grasp opposite corners
Grasp the top corner with one hand and the opposite bottom corner with the other hand.
This causes the object to come to a resting position in the Power Zone during lift and reduces
the potential to use a jerking motion. Avoid grasping by straps and bands as they might break
or result in a cut/laceration to the hand.
6. Lift with a smooth and steady motion
Lift with a smooth, steady motion-Dont jerk: Let all the muscles work together when
lifting and/or lowering an object.
7. Move your feet
Step or pivot; dont twist: Feet are used to change direction. If you try to change
direction without moving your feet, you are forced into an end-range position. To avoid a twist,
take at least one step.
8. Use equipment to assist in the lift or lower
Look for opportunities when existing equipment and/or facilities can support all or most
of the weight of the object being handled. The less time you spend bearing the weight of the
object the better.

Slips and Falls

-Use Safe walk paths
Eyes ahead of work, scan work area, and use designated walk paths. Use three points
of contact with equipment and laddersthis means you must keep two hands and one foot in
contact with the equipment or ladder while you move the other foot; or you must keep two feet
and one hand in contact with the equipment or ladder while you move the other hand. If you
slip, you will be better able to stop the fall.

Pushing and Pulling


Think Power Zone
The Power Zone is the area within your range of motion that gives you the maximum
strength capability with the most comfort. The Power Zone for push/pull is with the hands in
front of the body positioned from slightly above or near waist level up to the armpits.
-Know your Power Zone
To get your body in position to stay in your Power Zone, follow these guidelines:
Feet: Shoulder-width apart, one foot ahead of the other.
Torso: (Push) Lean slightly forward throughout the movement. (Pull) Lean slightly backward.
You may need to lean more to start the motion.
Arms: Bent slightly (not locked).
Hands: In front of the body, at or slightly above waist level.
Shoulders: Square shoulders to the direction of movement you plan to create.
Rule of Thumb: Align yourself with the object and the path in which you want the object to
travel.
-Minimize end-range motions by improving body position
End-range motions occur at extreme, end-limits of a body parts movement. For

required.
-Secure footing
Sturdy work shoes with slip-resistant soles will prevent/reduce slips and falls on the
various surfaces encountered.
-Slide, walk, or roll bulky objects
Working within your limitations, slide, walk (move one side at a time), or roll the
object. Always use existing equipment when available to assist you in pushing/pulling, seek
assistance when necessary.

Powered Equipment
Be aware of your surroundings. If it can move, it will.
-Yard control
Only certified personnel are allowed in the yard outside of the designated pedestrian
walkways and break areas.
-Safety as a pedestrian
Stay within designated walk paths when in the operating area of powered equipment.
Designated walk paths may provide slip resistance, protection from powered equipment, and
safe egress.
-Conveyors
The purpose of conveyor systems is to move packages. Do not climb on conveyor
systems. Wear clothing, jewelry, and hair styles that prevent entanglement with conveyors and
other equipment. Keep fingers, thumbs, and feet away from belts and drive rollers. Stay out of
restricted areas.

Plan for the Unexpected


If it can happen, it will.
Although there is no way to prevent every unexpected event from occurring, we can
take steps to minimize the risk of injuries during such events.
-Have a set routine
-Use designated walk paths
-Place objects securely
We consider that an object is placed securely when the bottom is fully supported by the
surface.
-Face your work
Standing at a 30 to 45 degree angle to the flow of packages allows you to see the work
that is coming to you and where the work will be going. Always keep your peripheral vision
clear to enable your eyes to perceive the widest angles.
-Open doors cautiously
Open overhead doors cautiously and report if difficult to open. Be prepared to move out
of the way of falling objects that begin to fallyou do not yet know the weight or composition
of the objects and it could result in injury.
Know location of emergency equipment
-Know emergency response plan
Everyone should be familiar with the work area and the emergency evacuation plan to
allow them to safely get out of the area in case of an emergency.
-Maintain adequate egress
In all work tasks and areas we need to maintain a safe path or egress (at least 18 inches.)
-Be aware of sharp and splintery objects
-Keep work area clean
A clean-as-you-go philosophy allows you to work more efficiently and safely.
