RELEASE OF LIABILITY

for my son/daughter/myself to attend and participate in the casting sessions of (name of

I acknowledge that I will not seek to have (name of entity organizing the casting call)
or (Name of entity that owns the property casting is hosted on) held liable in the event
that any accident, injury, loss of property, or any other circumstance or incident occurs
during or as a result of my sons/daughters/my own participation in the event. This
release of liability includes accident, injury, loss, or damages to my son/daughter/myself,
as well as to other individuals or property which may result from my sons/daughters/my
own participation in the event. I hereby release and agree to hold harmless (name of

is hosted on) and its officials, agents, employees, and volunteers, from any claims arising
out of my sons/daughters/my own participation in this event.
