Memorandum of Agreement (reconstructed)

KNOW ALL MEN BY THESE PRESENTS:

This Contract is made and executed this 15 th day of February 2015 in Labo, Camarines Norte by
and between:
MARIVIC C. ALOC, T-III Tulay Na Lupa National High School (TNLNHS) Proponent/Coordinator
Of Because We Care: We Find Ways an existing Income Generating Project organized to
support the needs of student beneficiaries of the weekend tutorial program with office address
at Tulay na Lupa National High School, Tulay na Lupa, Labo, Camarines Norte herein referred
to as the PROPONENT;
- and
TULAY NA LUPA NATIONAL HIGH SCHOOL (TNLNHS), with office address at Brgy. Tulay na
Lupa, Labo, Camarines Norte, represented herein by Mr. DANIEL B.LANIT, Secondary School
Principal II hereinafter referred to as the "ADMINISTRATION".
WITNESSETH:
WHEREAS, MARIVIC C. ALOC, as authorized by the Department Education, is engaged in the
provision of printing and photocopying services that benefits majority of the TNLNHS populace;
WHEREAS, the ADMININSTRATION agree to support TNLNHS Because We Care : We Find
Ways Income Generating Project through a Project Proposal approved by the Schools Division
Superintendent on July 4, 2014;
WHEREAS, MARIVIC C. ALOC was granted by her mother in law a personal loan amounting to
one hundred twenty thousand pesos (P100,000) to be used in the purchase of a riso graph
machine and its consumables from Copylandia Office Systems Corporation;
NOW, THEREFORE, for and in consideration of the above premises and of the mutual
covenants hereinafter set forth, the PROPONENT and ADMINISTRATOR do hereby agree as
follows:
1. MARIVIC C. ALOC shall utilize the personal loan of one hundred thousand pesos
(P100,000) to purchase of a riso graph machine and corresponding consumables;
2. MARIVIC C. ALOC, shall guarantee the payment of monthly amortizations of the
corresponding loan by charging rental fees in every transaction;
3. MARIVIC C. ALOC, shall apply for business registration in the barangay, mayors office,
the DTI and the BIR as sole proprietorship with the name BWC Printz to ensure the
legality of its existence in the school as it will continue to subsidize the cost of printing
and reproduction of documents for the benefit of the school especially its student
populace;
4. TNLNHS ADMINISTRATON shall course the printing and photocopying of the
documents pertaining to its operations subject to the usual accounting rules and
regulations set by the Commission on Audit (COA);
5. TNLNHS ADMINISTRATION shall endorse the application for tax exemption of BWC
Printz in line with the Department of Educations Adopt a-School Program as it will
provide subsidize the printing of reading manuals to be used in the assessment,
remediation and evaluation of the students reading skills and performance particularly in
Grade 7; .
6. MARIVIC C. ALOC, TNLNHS BWC : WFW proponent and now owner of BWC Printz
shall directly manage the project in coordination with Mr. DANIEL B. LANIT, Secondary
School Principal II and the TNLNHS Financial Staff;
7. MARIVIC C. ALOC shall solely take charge of the renewal of the business permits with
its corresponding financial requirements and fees;

8. Efficient documentation, reporting and monitoring of the project is a responsibility of

MARIVIC C. ALOC, the proponent; as the ADMINISTRATION shall validate and
countercheck the correctness of such.
9. This Contract shall take effect for a period of 12 months commencing on March 01, 2015
and ending on March 01, 2016 subject to renewal.
IN WITNESS WHEREOF, the parties have signed this Contract on the date and at the place
above-written.

voluntarily signed the foregoing Memorandum of Agreement which she acknowledged before

BWC: We Find Ways Program., and that she has the authority to act in such capacity.
The foregoing Memorandum of Agreement consisting of two (6) pages including this page on
which the acknowledgment is written has been signed on the left margin of each and every
page by the parties and the witnesses.
