Notes: Working on one goal at a time until Hayden can bear weight on her legs. Working with the
crawling position first.

Impression: We went outside this time to use our probe test with Hayden. We had
a blanket for her to be on and a pool noodle to work with her on. She was not as
interested in the pool noodle as the time before. She had a difficult time crawling
on the grass even with a blanket down. When we went back inside, Hayden did
much more crawling with assistance and was more happy.
Objective Description of Data and Observations: We prompted Hayden to move
forward towards the noodle by modeling, verbal direction, and by partial and full
physical assist. We did the same with the noodle underneath her. The first time
Hayden moved forward to steps but started to cry. The second time she moved
forward more and played with the noodle when she reached it. The third time
Hayden did not move forward and continued crying. Inside, Hayden would crawl
forward with physical assist several steps.
Assessment and Evaluation: Hayden had just woken up from a nap, it took a little
bit for her to warm up to us and want to play. Once Hayden was up and ready, she
was able to move forward on her own moving her arms, and one leg. We would
assist her in helping her move forward and feel the motion. As a team we can read
each other's cues of what we need to next. We work smoothly together and
encourage each other when helping Hayden.
Plan: This was our last session with Hayden. We wanted to leave the parents and

family with ideas to help Hayden. We reviewed with the mother different activities
that she could do with Hayden. We reviewed with her new ideas that she could
use and to try with Hayden to help bear weight on her legs.
