Policy:

The Company will provide for a primary and secondary centers of


operation equipped with appropriate supplies and equipment.

Purpose:

To prepare for safety and recovery as a result of a disaster.

Scope:

This statement will apply to alternative centers of operations and


recommended supplies.

Procedure:

1.0 SUPPLIES AND EQUIPMENT


The following supplies and equipment shall be stored at the primary and
secondary Centers of Operations, and at each branch, department and facility
as appropriate:

Current full copy of the company's Disaster Management Plan and


operations procedures for each branch, department, facility and
function.

Building plans for each facility, including primary control points for all
utilities, if applicable.

Complete and current list of all personnel, including names, addresses,


and phone numbers. Current position descriptions indicating routine
duties and responsibilities shall be included.

Complete and current list of all equipment and serial numbers;


replacement information; names and phone numbers of designated
vendors; and service agreement information.

Complete and current list of all emergency services, community


resources and related agencies within each county, or other
geographic region, serviced by the company.

Food, water and medical supplies as appropriate.

Battery-powered radios capable of receiving emergency instructions;


fire extinguishers; flashlights; and basic tools as appropriate.

Master keys, alarm access codes and other security information for
each branch, facility and the other Centers of Operations.

Stationery items, numbered forms and negotiable documents as


appropriate.

Assorted writing materials, instruments and business tools as


appropriate.

Non-electrical mechanical devices as appropriate.

Chronological log forms to document all actions taken during the


emergency.

Any other items considered appropriate during the initial and annual
review process.

2.0 STORAGE SITE


In addition to the primary and secondary Centers of Operations, a storage
site may be maintained at a location distant from those Centers. This storage
site may contain items acquired in addition or in place of the items stored at
the Centers of Operations. These items are to be determined by the Disaster
Management Team Coordinators. This storage site is:
