I, TERESITA CHUA TE, Filipino, single, of legal, age and a resident of No. 502 R.
Pascual Street, San Juan M.M., after having been subscribed under the Law, do hereby
depose and state that:
1. Today, 29th day of January 2002, when I came to the office of the ECE REALTY &
DEVT., INC., I found out that my Contract to Sell for Fantasy World Theme Parks,
Amusement and Recreation Club, Inc. FW# 4632 was given to my agent, Heidi Chua.
2. However, the said Contract to Sell was never delivered to me by my agent.
3. I am executing this affidavit to attest to the truth of the foregoing and for whatever
legal purpose this may serve.
