Situation: The following dialogue between a nurse and a patient whos

just being admitted to hospital. The nurse is taking personal details and
asking about the patients general state of health.
Nurse

: Good morning Ms. Umi

My names Rita, Im a registered nurse, and Ill be
looking after you today. How are you feeling now?

Patient

: Good morning. Im feeling alright just now, thank you.

But Im feeling a little nervous about being in hospital.

Nurse

: Dont worry, theres nothing for you to be concerned

about.
Now Ive got several questions I need to ask you, and
some paperwork to go through, for your admission. Are
you ready?

Patient

: Yes, I suppose so. What do you need to ask me?

Nurse

: Firstly, Id like to check your personal details.

Patient

Nurse
: Are you still living at the same address? Harapan Indah 2
block D no. 8 ? Could you tell me your date of birth, and your religion?
Patient
: Yes, I still live at that address. I was born Februay, 1965.

Nurse
: Can you give me a few details about your medical
history?
Have you or any close relatives ever had hypertension,
diabetes,
asthma, heart disease, cancer, or any operation?
Patient

: I had high blood pressure 7 years ago, and nearly two

I was diagnosed with diabetes. Im controlling for high
blood pressure very well with diet and exercise, but I
cant controlling for my diabetes mellitus.

I also had a heart attack 5 years ago. I had to have a heart

operation.
Nurse

: And whats the reason youve come into the hospital

Patient
: When I was walking to the shops to buy the paper this
morning,
I suddenly got a tight feeling in headache, reduced visibility, easy weak,
easily hungry, thirsty easily.
I thought it was a good idea to come to the hospital.
Nurse

: Are you taking any medications?

And are you allergic to any drugs or foods?
And do you smoke or drink?

Patient
: Im only taking some vitamins, and as far as I know, Im
not allergic to anything.
I gave up smoking 5 years ago, after my heart operation, and we
only drink a little wine on special occasions.
Nurse

: Are you in any private health fund?

Patient

: No, were not in any health fund.

Nurse
: Ill just take your temperature, blood pressure, blood
sugar, physical examination like weight, blood sample, and others. And
then get you to write down the phone number of your next of kin.
The doctor will come and see you in a few minutes.
