when I got home and called the doctor for a blood test. We had only one

I was so worried that one of us was going
to be sterile that I told him to get a fertility
test. The next day I found out I was pregnant! So when he got home from work, I
asked if he had picked up the test yet. (He
hadnt.) Then I really casually said, Well,

I took one test before work but didn't believe
it was really positive, so I didnt say a peep.
We were having a party that night, so while
my husband went to the store to get a couple
things, I went to the store, came home and
took another test. Two positivesthere was
no denying it now! I wrote a note that said,
Guess who is going to be a daddy? and set
it on the table next to the two tests. It was
the first thing he saw when he walked in the
door, and I still cant believe he didn't drop
the six-pack in his hands when he read it. I

were both giddy keeping our secret from

After our second round of IVF, I found out
I was pregnant while my husband was in
New York City. I picked him up from the

Days gone by

thought that flitted through my brain when
I sat down to write this note: December?! You mean
2016 is almost over? Wasnt it January like five minutes
ago? Turns out our grandmothers were right, you

Heres where most people would encourage you
to enjoy every second because it passes so quickly
but I live by a different philosophy. Honestly, I dont
enjoy every second. Im coming off a week filled

even before your

Think about how your dog acts when
something unexpected happens or a
newcomer enters your home. Is he easygoing? Hyper? Anxious? Every dog is
different, attests Hodgson, but you know
your canine counterpart best, which is
why you might be able to predict how
your pet will respond.

Play pretend
The best thing to do to prepare a dog for
the reality of life with a baby is to bring a
baby home now, Hodgson counsels. Not
the baby, of course, but a doll. Sure itll be
made of rubber, but holding it, rocking it,
and treating it like its the real thing will
help acclimate your pup. Expert tip: If you
have a friend with an infant, ask to borrow
a blanket, and while youre at it record a
tantrum or two. Get your dog socialized
to the sounds and smells of a real baby,
suggests Hodgson.

It can be harder to get pregnant
the second time around, but
there are steps you can take to
beat the odds. BY PAMELA BRILL

You might be diagnosed with


secondary infertility if ...
division, explains Philip Chenette, MD,
fertility specialist at the Pacific Fertility
Center in San Francisco. A woman trying
to conceive her second child at age 40 will
find her fertility half as good as compared
to age 38.
Jill Hechtman, MD, an OB/GYN in
Tampa, Florida, agrees: As we age, there
is a natural decline in the number and
quality of our eggs. We are born with all
the eggs our bodies will ever make.
Hechtman compares a healthy 30-yearold with a 20 percent chance of becoming
pregnant each cycle to a 40-year-old
whose odds drop to 5 percent.
With age also comes an increased risk
of other complications associated with an
older uterus: fibroids or polyps, irregular
or lack of ovulation, scar tissue or chronic
endometritis (an inflammation of the
uterus). There is also increasing concern
about high blood pressure and diabetes,
which become more common as the
kidneys and heart age, adds Chenette.

High-risk concerns
If your first pregnancy was considered
high-risk, you may wonder if that sets
you up for successive complications. The
answer: not necessarily. According to
medical experts, high-risk pregnancies
do not directly affect future fertility. Just
because you were labeled high-risk the
first time doesnt guarantee that thats
how things will play out in round two.
Instead it can serve as a tip-off to prevent
related complications.

A mans age does not come into play when determining the cause for secondary infertility.
Men have far more fertility reserve and continuously produce sperm throughout their
reproductive lifespans, notes fertility specialist Philip Chenette, MD. A poor diet, stress,
smoking and heavy alcohol consumption become more of a concern as men age,
contributing to a decline in sperm concentration and motility.

ow that your firstborn is getting


older and you feel like you have
gotten the hang of motherhood
(finally!), you might be ready to expand
your family once again. But, after months
of negative pregnancy tests, you begin to
wonder: How can it be this hard? Weve
done this before. Its not like Im infertile.
Having one baby doesnt guarantee any
mother that shell be able to easily conceive
again when shes ready. In fact, if you find
that getting pregnant is proving to be more
difficult the second time around, you arent
alone. According to the Centers for Disease
Control and Prevention, 830,000 American
women ages 15 to 44 experienced secondary
infertility from 2006 to 2010.
Although a number of factors can
contribute to trouble conceiving after having
one child, experts believe that there are
viable ways to boost your chances of having
a successful subsequent pregnancy. By
taking a closer look at the common causes
and zeroing in on how to overcome them,
concerned couples can put their fears to rest
and focus on growing their broods.

What [a high-risk pregnancy] may do


is suggest the next pregnancy would be
high-risk as well, explains Mary Jane
Minkin, MD, clinical professor of obstetrics,
gynecology and reproductive sciences at
Yale University School of Medicine. For
example, the No. 1 risk factor for preterm
labor and delivery is indeed previous
preterm labor. Minkin also notes that
prior gestational diabetes produces a greater
risk of developing repeat gestational
diabetes. However, as Minkin points out,
this concern can be addressed in advance.
If a woman is overweight and loses the
weight, she will reduce her risk.
Other previous conditions that could
serve as warning signs for future pregnancies
include a uterine septum (a malformation
of the uterine cavity), an incompetent
cervix (cervical inefficiency) and fibroids,
which can become progressive.

Getting a boost

Once you have ruled out possible medical


concerns, experts recommend a number
of ways to actively boost your chances for
another positive pregnancy test. For starters,
add at least 400 micrograms of folic acid to
your diet before conceiving. It minimizes
the chances of neural tube defects, says
Minkin.
Another tip? Maintain your body weight
as best you can. Both too high and too low
[weight] can make women not ovulate
well, Minkin says. A sensible, balanced
diet is also key to providing your body with
essential vitamins and nutrients.
Making important lifestyle changes in
fitness and sleep can help improve your
chances for conception. Fertility rates
decline at a rate of 2 to 3 percent per month
which, over time, becomes a significant
barrier to fertility, explains Chenette.
He advises cutting down on your alcohol
intake, quitting smoking and getting
plenty of rest, along with keeping your
BMI under 30. If you feel you need to
lose weight, seek guidance from an
experienced nutritional consultant, and
balance exercise and calorie intake to
keep your body in check, he adds.

Standardized tests
A variety of prescreening options can
alert women to possible fertility issues in
advance. Blood tests [can] check for
ovarian reserve and function, but [they]
have not been proven to reliably predict
the possibility of becoming pregnant,
cautions Hechtman.

A healthy diet is a must when
youre trying to get pregnant.
Boosting our fertility is really

the uterus, follicles and eggsare

baby, says Ebeth Johnson, the nutrition

To up the odds of a positive
pregnancy test in your future, try adding
these good-for-you eats to your plate.

WHOLE FOODS: Free from


additives and minimally

and fiber. Organic meats
and produce lessen the
amount of toxins that are
introduced into the body,
notes Johnson. A body with
a lower toxic load is more
likely to conceive a baby.
Add brown rice, wholewheat pasta and quinoa
to your shopping list.
FRUITS AND VEGGIES:
Rich sources of vitamin C,
potassium and antioxidants,
plant-based foods are
best when eaten in season.
Dark green vegetables like
spinach, Swiss chard and
romaine lettuce are packed
with nutrients that support
follicle development and
egg release.
LOW-FAT DAIRY: Yogurt,
fat-free cheese and skim
milk provide protein and
calcium, which are necessary for maintaining bone
health. Topping off Greek
yogurt or cottage cheese

with berries, fruit or nuts


makes a sweet treat without
any added sugar.
LEAN MEATS AND
BEANS: Also good sources
of protein and calcium,
these foods offer the added
benefit of iron. Increasing
iron levels encourages
ovulation and can help

daily decisions can be even more overwhelming when youre on a modified diet.
If youre watching your carbs because
of gestational diabetes, eating gluten-free
due to an allergy or intolerance, or simply
skipping meat or dairy for personal
reasons, weve got you covered. After
chatting with a few experts, were giving
you the lowdown on what you should
know about common diet restrictions
during pregnancy.

A gluten-free diet is the primary treatment


for celiac disease, an autoimmune condition
in which the body reacts to the gluten
protein found in wheat, barley and rye by
attacking the intestines. According to
Alaina Strobel, RD, LD, CDE, at Texas
Childrens Hospital Pavilion for Women
in Houston, this results in gastrointestinal
damage and malabsorption of key nutrients
which is why people with celiac disease
are advised to avoid foods with gluten
altogether.
Outside of celiac disease, going sans
gluten has increased in popularity in part
because some have touted its overall
healthfulnessalthough the experts insist
there are no extra health benefits to going
gluten-free unless your body has shown
a negative reaction to gluten.
If youre pregnant and eating glutenfree, you should know that its absolutely
possible to get the necessary nutrients

as potatoes, rice, corn, nuts, beans,
lentils, eggs and dairy can also be used
to supplement the loss of nutrients from

To up your fiber and overall nutrient
intake, Vandana Sheth, RDN, CDE, a
spokesperson for the Academy of Nutrition
and Dietetics, recommends focusing on
naturally gluten-free whole grains such as
brown rice, quinoa or amaranth instead
of heavily processed gluten-free grain
products, which are often stripped of
key nutrients and fiber.

certainly not alone. According to the Centers


for Disease Control and Prevention,
gestational diabetes affects up to 9 percent
of expectant women.
With GD, your pregnancy triggered the
diabetes, and the condition will likely go
away after youve given birth. Until then,
your blood sugars run higher than normal,
and if left unchecked, can lead to serious
risks for both you and your baby.
Babies born to women with uncontrolled diabetes tend to be larger, tend
to have a large amount of amniotic fluid,
and tend to have complications after
birth, including an inability to regulate
their own blood sugars, says Bardach.
However, diet plays a huge role in GD
blood sugar management, and thankfully
theres lots you can do to avoid those
complications.
Rather than thinking of it as a restrictive
diet, Bardach likes to refer to GD-specific
eating as a healthy eating plan that is rich
in nutrients and low in fats, calories and
carbohydrates. What does that look like?
Lots of fresh fruits, vegetables and whole
grains. Instead of simple sugars in baked
goods or processed foods, you should
choose complex carbs such as oatmeal,
brown rice and whole grain pastas.
I will generally tell women, once
diagnosed with gestational diabetes, that
a great way to start this diet is to replace
all white foods with their brown alternatives, she says. So ditch the white bread
for whole wheat, white rice for brown,
and russet potatoes for sweet potatoes.
Forgo the fruit juice or soda for some
sparkling fruit-infused water, and youre
well on your way to a diabetes-friendly
diet.
As Strobel points out, this way of eating
is less about what to eat and more about
how much. So get out your measuring
cups because portion control reigns queen
here, particularly when it comes to carbs
(the main culprit in blood sugar spikes).

Just how many carbs you shoot for in


any given meal will depend on multiple
factors, so ask your doctor if you dont
already know those specifics. But in general,
Sheth says most women with gestational
diabetes can eat between 30 to 45 grams
of carbohydrates per meal, and up to 15
grams of carbs per snack.
The more protein and veggies you load
your plate with, the more satisfied youre
going to feel. When in doubt, always add
a protein to your carbs. For example, snack
on a piece of string cheese with that apple.
According to Sheth, its that carb-protein
combo that helps to blunt the rise in blood
sugar.

Vegging out
Many people today follow a wide variety
of vegetarian and vegan dietsfrom
vegetarianism (which excludes red meat,
poultry and seafood) to ovo-vegetarian,
(which allows the consumption of eggs)
to pescetarian (which includes fish but no
meat or poultry). Veganism is the most
restrictive, excluding all animal products,
including meat, dairy, eggs and honey.
Religious leanings, health goals or ethical
considerations the reasons someone
might follow one of these diets are as
varied as the restrictions themselves.
As far as pregnancy concerns go, all of
these diets can be safe for mom and baby.
The trick is in knowing which nutrients to
pay extra attention to and ensuring youre
eating an array of whole foods to get those
nutrients.
Most vegetarian diets will meet most of
the nutritional needs and goals for pregnant
patients with the exception of iron, vitamin
D, vitamin E and chloride, says Bardach,
who notes that focusing on plant-based
proteins such as grains, legumes and nuts
can provide the vitamins and minerals
necessary for a healthy pregnancy.
Make sure youre meeting your
increased protein requirements (70 to

90 grams daily) by leaning on nuts, legumes,


beans and quinoa. For calcium and
vitamin D, look to orange juice, almonds
and broccoli. Dark leafy green vegetables
such as bok choy and collard greens are
also high in calcium.
To up your iron intake, Strobel points
to plant sources of iron (called non-heme
sources), including grains, spinach and
beans. Theyre best absorbed when
combined with a source of vitamin C,
such as citrus or strawberries, she says.
Conversely, If you need to take an
additional iron supplement, avoid eating
calcium-containing foods at the same
time because this inhibits absorption,
Strobel advises.
For vegans in particular, vitamin B12
and essential fatty acids are two of the
biggest nutritional concerns. Vitamin
B12 is obtained from animal sources
so a supplement may be necessary, says
Bardach. Your prenatal vitamin should
offer at least some of the vitamin B12 you
need, and many foods these days are
fortified with it as well. Check out the
food labels on cereals, soy milks and
other products to see how much youre
consuming.
Flaxseed, walnuts and soybeans can
provide the necessary amounts of DHA,
but you should talk to your doctor about
microalgae supplements if youre worried
that youre not getting enough from
plant-based sources alone.
One great veggie-friendly meal idea
for pregnant women: vegetarian chili. Full
of beans, vegetables, spices and tomatoes
(which enhance the iron absorption from
the beans and lentils), its easily customizable to what you crave and whats in
season.
Pour that on top of quinoa or brown
rice, says Sheth, or eat it with a whole
grain tortilla, and thats a great nutritionally balanced meal. P&N

Easy as 1, 2, 3
Labor can be broken down into three stages.
The first consists of early labor, active labor
and transition. This is the time during
which contractions take place, and your
cervix thins out (effacement) and opens
(dilation) to prepare for the big event. Stage
two happens when you are fully dilated
and actively working to push baby into the
world. The third and final stage takes place
immediately after babys birth and ends
with the delivery of the placenta.
The duration of each stage is anyones
guess. Average estimations for the total
length of labor for first-time moms fall
somewhere between 16 and 20 hours, but
the truth is, it can vary greatly in either
direction.

First up

all too obvious that your body is undergoing some
pretty remarkable transformations in preparation
for babys debut. Although the bump is the most obvious
change, whats going on insideand out of sightis
perhaps even more stunning. And it all culminates in the
final frontier of pregnancy: labor. Heres a look at what
happens during each stage of the process, from the Wait,
was that a contraction? to the big Hello, baby! at the end.

Early labor. Its a little different for every


mom, every time, but most women know
theyre officially in early labor when they
begin feeling contractions at somewhat
regular intervals. Although you might be
experiencing discomfort (it can range
from bothersome to painful), most care
providers encourage mothers to hang tight
at home in early labor. (Note: If youre
high-risk, preterm or experiencing
complications, go ahead and make your
way to the hospital for safetys sake.)
Many moms opt to stay active during
this time. Light walking can keep your
mind off things (plus, it can help labor
progress), and you might also be taking
care of a few last-minute tasks, like

In the last month of your pregnancy,


youll be seeing a lot of your care provider
most obstetricians and midwives like
to see their patients once a week upon
reaching the 36th week of gestation. At
some point, a physical check down below
is typical, and your practitioner might
report that you are dilated and/or effaced
to a small degree. For a gal looking to get
the show on the road, thats great news
but dont get too excited just yet. Although
its a good sign that your body is on the
right track, it doesnt necessarily mean
baby will be making her grand entrance
the next day.
Your first real sign that baby is on her
way will most likely come by way of
contractions. Theyll feel similar to the
Braxton Hicks contractions you had in your
final trimester, but they might not hit hard
and fast right off the bat. For many moms,
labor starts with a kind of crampy, uncomfortable feeling that eventually leads to the
moment of realization: This is the real thing!
When that happens, youre in early labor.

EXIT STRATEGY
putting the finishing touches on your
delivery bag or tossing one last load of
laundry into the dryer.
If you need some relief, try getting wet.
Water is a wonderful comfort, says
Margaret Buxton, CNM, clinical director of
Baby+Company birth center in Nashville,
Tennessee. So, consider hopping in the
shower or soaking in a warm tub. Buxton
suggests setting the mood, too: Keeping
the lights dim, listening to good music,
and eating light snacks are also helpful
comforts.
Eventually, youll notice that the
contractions have become longer, stronger
and closer together. This is normally your
sign that its time to hit the road. If you
havent already checked in with your doctor
or midwife, put her on alert. Buxton and
her fellow midwives at Baby+Company
encourage families to call and check in with
them during early labor; some doctors
might prefer you to call when your
contractions are occurring consistently
in a specific pattern, such as five minutes
apart. Ask your provider at one of your
prenatal appointments when shed like to
be notified. If youre ever in doubt, dont
hesitate to pick up the phone.
At some pointnormally when your
cervix has dilated between 4 and 6
centimetersyoull notice that the tone
of your labor changes. That means youve
moved on to the next phase of stage one:
active labor.
Active labor. Most women easily recognize
when they move into active labor. Active
labor brings a change in intensity, explains
Buxton. Not only will your contractions
grow stronger, but many women feel the
sensations deeper in their pelvises and
down into their groins, she says.
Dealing with these contractions might
be more challenging than those of early
labor. Some women opt for pain medication
or an epidural at this point in the game;
others continue with the same natural
methods they exercised in early labor.
Breathing techniques can be useful, and
mobility is still a great help, although youll
most likely find yourself leaning against a
wall or your partner during contractions
rather than walking through them, as you

could before. Rocking or bouncing can


provide further relief.
The changes wont just be physical,
though. Mothers mood often changes as
well, says Buxton. Going from cheerful
and talkative to quiet and focused during
a contraction is a sure sign that she is
settling into active labor.
Transition. When your cervix opens from
8 to 10 centimeters, youre in whats often
considered the most challenging part of
labor: transition. The contractions are real
here, ladiesvery intense and right on top
of each other. You might shake, shiver,
feel nauseated or vomit. Transition is the
most intense part of labor, and it takes
everything a mom has to stay focused and
present in the experience, says Buxton.
Your previous methods of pain
management might not be as effective
here, and you might get pretty snippy.
(No worries, everyone will forgive you!)
According to Buxton, Women often
become very focused on each contraction,
are easily annoyed by distractions in the
room, may not want to be touched, and
need a lot of support from their providers
and support people.
As your baby moves lower into position,
youll feel intense pressure in your rectum.
Some moms who are delivering drug-free
have an undeniable urge to push when this
happens, which escorts you right into the
next stage of labor. This is where things
get exciting.

Second act
Once your cervix is fully dilated, its go
time. Youll work with your contractions,
bearing down during each one, to move
baby through the birth canal.
When it comes to pushing, one size
doesnt necessarily fit all. Your caregivers
might walk you through a counting and
pushing routine. (This is almost guaranteed if you received an epidural because
you wont have full feeling down below.)
Others might encourage you to push when
you feel ready. The more you allow your
uterus to work and move baby into position,
the less physical exertion you have to put
inbut it can be hard to wait when you
feel the urge to bear down.

Blood bath
You may notice an increase in mucous vaginal discharge leading up to and during labor,
which is normal. It may be tinged with blood, which can also be normal but is worth a
mention to your caregiver. This bloody show is a good indicator that youll soon be
meeting your babybut soon is a loose term here because, like with everything labor
related, it could still be hours, days or even weeks (sorry, mamas) before that final push.

Your cervix prepares for babys arrival in two


ways: (1) dilation, the opening of the cervix,
and (2) effacement, the thinning of the cervix.
Dilation is measured in centimeters, while
effacement is measured by percentage. Its
delivery time when youre 10 centimeters
dilated and 100 percent effaced.

Pushing is tiring, but its also a really


encouraging time because youre finally
able to take action. You have a sense of
control now that you didnt have before.
As you continue to work on moving baby
down, his head will become visible. The
pressure on your perineum will be intense,
and youll experience that ring of fire
that means baby is almost here. Your midwife or OB might guide you through the
next few pushes to help minimize tearing.
Babys head will make its appearance,
followed by his body, and youll finally
come face-to-face with the most amazing
person youve ever met. Emotions will be
high! Soak up that sweet baby, and allow
your little one to latch on for his first
breastfeeding session if youre up for it.
Babys cord will be cut shortly after his
birth, but the timing might vary depending
on whether youre delaying cord clamping
and/or storing babys cord blood.

Threes company
The third stage brings both good and bad
news. The bad news is your job isnt done
just yettheres still a placenta to be
delivered. The good news: Its a whole lot
easier than delivering the baby.
The placenta is soft and squishy, so it
wont cause any tearing or pain, assures
Buxton. [It] usually comes out between
five and 15 minutes [after baby], but sometimes can take up to 30 minutes. Providers
will usually give the cord a gentle pull to
see if it has released from the uterine wall.
If it has, gentle traction and a small push
from the mother will allow it to deliver
easily.
Once placenta delivery is complete,
youre officially in the recovery zone. If
your care provider has any work to do
down below (like stitching up a tear),
shell take care of it now. Otherwise, your
uterus will continue contracting as it
begins shrinking back to its normal size.
It takes a bit of time, so youll feel crampy
afterbirth pains in the days following
labor. Rest assured, though, they are
nothing compared to what you just went
through. Youve conquered the hard part,
and now youre on to the next (and best)
stage of the whole ordeal: motherhood.

remember. Maybe everyone would forget.
Everyone but me.

Take two
When I found out I was expecting my
second child, I decided to be lighthearted
about the birth plan. I wasnt going to have
onesimple as that! Why go through all
that planning and daydreaming for it to
end up differently? I took a practical
approach and had the mantra of que sera
serawhatever will be will be. Secretly I
still wanted my natural birth story. Not
only for my selfish reasonsbut for my
child. But did I dare put myself and my
husband through that again?
Fast-forward (because that second
pregnancy really flew by at lightning
speed) to week 39. This marked the start
of biweekly stress monitoring and ultrasounds to measure my fluid levels in the
sac. I had shared at my routine checkup
that movements had slowed, so my doctor
ordered testing to ensure the health of our
little boy blue. It was nothing new to us
we had been through the same routine
with our daughter.

However, with our daughter, we only


ended up having one appointment before
her birth. So when we strolled into the
office for the fifth time with our son, it
was all new, and the anxiety was growing
greater by the day.
I was 40 weeksand threedays. We could
sense something was off this time because
the ultrasound technician excused herself
from the room to grab a doctor. As if our
sharp glare toward each other wasnt
enough, my husband and I said aloud,
Something isntright.
The doctor told us that my fluid had
dropped from a 9 to a 7. (Normal range is
5 to 25.) To make matters more intense,
the umbilical cord was wrapped around
our little guys neck. Doc felt the best way
to proceed was to schedule an induction
for that day.
I was totally, utterly, full-body nervous.
(1)I did not want any complications with
baby boys health. (2)What if I had to have
a C-section? (3)Pitocin! (4)An induction
is not natural. (5) Was I ready for twochildren? (Admittedly, a little too late for my
fifth thought.) >>

begin on his birthday. It begins
threeyears prior2 years, 11
months and 2 days to be exact. Thats when
I gave birth to my firstborn, a beautiful
baby girl named Makena.
Like many first-time mothers, I had
everything mapped out in my mind about
how my daughters birth would take place.
I had never loved anyone or anything
remotely close to how much I loved this
growing child, and I wanted to ensure she
had a healthy entrance into life.
I prepped the entire 40 weeks and one
day of pregnancy for the perfect birth.I
had read books and blogs, spentcountless
hours practicing HypnoBirthing techniques,
and took natural birth classes (during
which my husband submergedhis hand in
a bucket of ice for oneminute to help him
understand the intensity of a contraction
the same hand hed use to rub my lower
back throughout labor). We watched what
my husband called bloody, scary, youarent-having-our-child-like-that videos,
took a breastfeeding course, bought a
balance ball, listened to pregnancy and
parenting podcasts, installed a contraction
timeronbothof our phones, and wrotea
birth plan. Everything wouldgo according
to plan or so I thought.
After 16 hours of labor without pain
control, I threw in the towel. I felt like I
was in a triathlonI was in the lead, but
then I lost to the person in second place
called Epidural.
However, 45minutes later I was holding
my daughter, and all thoughts of defeat
melted away as I stared into her eyes for
the first time, finally grasping the phrase
love at first sight. She was wide-eyed,
alert, strong and healthynot what I
expected herto be after an epidural.
Hours turned into days, which turned
into weeks. But my birth story was still
haunting. I replayed it constantly in my
thoughts. Would I have to share it with
everyone I told I was going natural? Maybe
I could hide behind my newborn and the
question wouldnt even arise? Maybe they

Letting go
Now! I blurted.
She rushed back and
checked my dilation.
You are having this
baby!
I noticed ateam
quickly hurrying to my
crying plea. There must
have been six, maybe
seven,of them. This
was way too sudden. It
had only been an hour
and 50 minutes since
my water had broken,
since I had endured
contraction after
contraction, since I had been stabbed in my
spine. This was really happening!
My doctors office is across the parking
lot, which was where my doctor was. We
cant get in touch withhim, but we do have
a labor specialist here in case you feel the
urge, the nurse told me. I looked up
at the labor specialist who said that if I
could wait for my doctor that would be
the best option.
Finally, after severalcontractions,
they were able to get in touchwithhim.
He advised them he would be in the room
in fiveminutes. My sweet husband kept
encouraging me, Only a few more
contractions until our doctor arrives
threemore contractions to go two more
contractions onemore And in the
doctor flew.
Alright, Lindsey. When you feel a
contraction, I want you to begin pushing.
Wait, what? Itwas happening! It was
happening naturally. The way I wanted.
The way I wanted. Thats the story of
Stanleys birth.

Its not practical to hold on to an idea


that may not be your path, your destiny,
your design. What is practical is to take
deep breaths and remember that everything almost always works out the way it
was intended to for you, your body and
your baby.
Parenting isnt perfect, nor does it have
a written plan. It takes on its own course.
Allow room for error, and smile or cry
tears of happiness when moments do
resonate with you. Most aspects oflife arent
perfectwhy should our birth stories be?
Why should parenting be? My children
are the only parts of mylife that are
unconditionally perfect. P&N

As I reflect on my attitude toward my


birth stories, I have learned a valuable
lesson. I have learned that when it comes
to parenting, things arent going to be the
way you dreamed. Nothing will match the
plan you have in your headduringthe nine
months when you first begin your journey
of parenthood.

I had already dilated to 5 centimeters upon


arrivingat my appointment where my child
was going to be evicted from my womb. I
was in no pain. I was a warrior or so I
thought.
It was 6 a.m., a long time from when
we scheduled the induction at 8 p.m. the
day prior. It was 10 hours later, andmy
doctor was orderingthe Pitocin drip to
be increased.
My doctor came to check on me again
at 8:30 a.m. and decided to break my
water. Boy, oh boy(pun intended), did
I begin to feel the contractions.
It brought me back to those intense
16 hours of labor pains with my daughter,
and I couldnt help but bluntly remark,
What the hell? I wanted the epidural.
We were friends now anyhow.
Gosh, what a production it takes to get
an epidural. I wasnt watching the clock or
anything, but it felt like forever. My friend
had sent me an inspirational quote that said
something to the effect of, Each surge
brings you closer to your baby. I tried to
focus on that and think about the end result.
However, the contractions were getting the
best of me. To be honest, I didnt even feel
the epidural. Thats how much pain I was
experiencing.
Apparently, I was lying slightly on
my left side, which caused the epidural
drip to not work properly. I could still
feel everything with numbness in only
my right upper thigh. What is that good
for? I demanded the anesthesiologist
return to my room.
He is in with another patient and then
heading to another room after that, so he
will be in shortly, explained the nurse.
Having to wait that long was atrue test of
patience. It felt like an eternity before he
arrived. He ordered more medicine and
checked the entry site. Nothing could be
done at this point. I was far too close to
having my boy to start another pipeline
running out of my spine.
Let me know when you feel pressure,
my nurse said over her shoulder as she
was walking out of the delivery room.

Safe dreams

New room-sharing recommendations encourage families to rest easy.

dadbut in a separate sleep space
can reduce babys risk of sudden infant
death syndrome (SIDS) by as much as 50
percent, according to a new report from the
American Academy of Pediatrics (AAP).
Its a stunning statistic and a strong
argument for roomsharing, but there are
still a few important guidelines families need
to follow to ensure theyre sleeping safely.

Give it time
The AAP recommends parents keep their
mini roommates in their bedroom ideally
until babys first birthday, or for at least the
first six months. Research shows the rates
of SIDS and other sleeprelated deaths,
particularly those occurring in bedsharing
situations, are highest in the first six months.

Proximity is key
Babys bed should be positioned nearby mom
and dads bed, advises the AAP. The closeness
allows baby to be within view and reach of
her parents, so they can readily feed, comfort
and monitor her throughout the night.

No kids allowed
Sharing the grownup bed with mom and
dad is a big nono for babies, warns the AAP,
because the pillows, sheets and blankets put
her at risk of suffocation, strangulation and

With breastfeeding moms, I usually advise to hold off on
pacifiers until they have gotten more comfortable with
latching and feeding their baby, notes pediatrician Russell
Horton, DO. Some babies never want a pacifier no matter
how they are fed, and that is OK, too. But before you nix
the nook entirely, keep in mind that pacifiers have been
linked to a decreased risk for sudden infant death
syndrome (SIDS).
Remember, too, that whoever giveth must also taketh
away. Prolonged pacifier and bottle use beyond 1 year of
age can affect tooth development and potentially lead
to cavities, cautions Horton. Pediatricians recommend
stopping bottles at a year, and I usually start trying to
wean the pacifier off between 1 and 2 years of age.

it is never too early to start the search, advises

Creek, Arizona. I would definitely start no
later than the beginning of your third trimester

prenatal care provider (ask friends and coworkers with kiddos about their experiences,

top picks), but youll have a whole new list

Consider all the things that are most
important to you, and then feel free to ask
about those things, Horton suggests. If

October or November, peaks between
December and March, and tapers off in
the springsometimes as late as May.

Preventing the flu starts with taking


the illness seriously. Doctors recommend
annual flu shots and commonsense safeguards as the best ways to keep both you
and your little one healthy this winter.
Unlike other viruses, influenza can
be a truly deadly infection. It is not to be
taken lightly, notes Stan Spinner, MD,
chief medical officer for Texas Childrens
Pediatrics. This is a bad bug.

f all the routinely recommended


inoculations for children, the flu
vaccine is the one theyre least
likely to be current on, according to a 2015
report by the National Foundation for
Infectious Diseases. Although the reason
why isnt clear, it could be because the
vaccine is required annually for ongoing
protection, and remembering the once-ayear to-do isnt always a priority for busy
parents. Or, it could be that the flu isnt
perceived to be as big of a deal as, say,
measles or rotavirus. But the truth is:
Influenza is serious business.

Caused by a virus, influenza is a respiratory illness often associated with fever,


cough, body ache and fatigue. Anyone can
get the flu, but according to the U.S. Centers
for Disease Control and Prevention (CDC),
pregnant women and infants are especially
vulnerablenot just to infection but to
severe and even deadly complications.

If your newborn isor will betoo young
to receive a flu shot, the best way to protect
him is to protect the people around him,
starting with yourself, says Carol J. Baker,
MD, executive director of the Center for
Vaccine Awareness and Research at Texas
Childrens Hospital. >>

Not a chance, says Stan Spinner, MD. The virus we give


in the flu shot is the killed virus. It cannot infect you.
People might feel a little sore and achy for a day or two,
but its just your bodys reaction to the virus causing
those mild symptoms.

Breathing easy

On average, some 20,000 young children


a year are hospitalized because of the flu,
according to CDC estimates. Because their
immune systems are immature, infants
are at increased risk of complications
such as ear or sinus infection, pneumonia,
dehydration and even organ failure.
Theres no way to predict whether
your child will experience severe complications, Spinner says. Its best to be
protected ahead of time. He and other
experts recommend that all babies and
children ages 6 months and older get a flu
shot each yearand that they get them as
soon as the vaccine is available, usually in
early fall.
Thats because timing is important,
especially when it comes to babies. For
one thing, it takes about two weeks for the
body to develop flu-fighting antibodies.
Second, if this is your babys first time
getting a flu shot, shell need two doses,
spaced four weeks apart, to be fully
vaccinated.

The flu vaccine is safe throughout


pregnancy. Not only can it help a momto-be avoid dangerous flu complications,
Baker adds, but it can guard her new arrival,
too. The mothers antibodies are passed on
to the newborn, she says, and can remain
effective two months or more after birth.
Mommy can give her immunity to her
baby. Its like a gift, says Baker.
Anyone else who will be regularly caring
for or spending time with your baby should
be immunized as well, recommends Jillian
Parekh, MD, attending pediatric physician
at Montefiore Medical Centers Family
Care Center in New York.
That includes grandparents and older
siblings. People might think its an
outrageous request, but you have to have
your infants best interests in mind,
assures Perekh.

Stop the spread


Besides vaccination, doctors advise that
the best way to guard against the flu is to
prevent it from spreading. Influenza is
passed person-to-person through coughs,
sneezes and, unfortunately, kisses.
Wash your hands often, and make sure
anyone holding your baby does, too, says

at Nova Southeastern University
in Fort Lauderdale, Florida.
Cover your nose and mouth with
a tissue when you sneeze or cough
and then throw that tissue away. We all
know people who are compulsive about
cleaning. Thats a good thing during flu
season, says Mulligan, who also serves
as chief medical officer of MDLIVE, a
telehealth company.
Of course, you should do your best to
keep your newborn away from anyone
whos sick. And if you come down with
the flu? That might mean dad or grandma
or grandpa have to pick up some of the
responsibilities, Baker says. But, she adds,
if youre up to it, breastfeeding is still safe,
even if youre taking antiviral medication.
Its also a natural way to boost babys
immune system.
No flu-prevention effort is 100 percent
effective. Even if youve done everything
you can, you or your baby might still get
sick. Call your pediatrician if you suspect
your little one has the flu, advises Parekh,
and know that symptoms can seem a lot

UNDERSTANDING RSV

ike the flu, respiratory syncytial virus


(RSV) often begins with symptoms
such as cough, fever, and runny or
stuffy nose. And, like the flu, its highly
contagious.
In fact, nearly all children will have
been infected by RSV by the time they
turn 2, says Deborah Mulligan, MD. For
most of them, the infection is no more
troublesome than the common cold.
But for some, RSV can become lifethreatening, leading to pneumonia or
bronchiolitis, an infection of the airways
that has been linked to asthma later in
life. Newborns are among those most
at risk for dangerous complications.

Unlike the flu, however, there is no


vaccine for RSV and no treatment. For babies
at extremely high riskthose who were born
prematurely or with heart diseasedoctors
might recommend injections of antibodies
that can help prevent infection during peak
RSV season. The injections are very expensive,
though, and only administered in special cases.
According to Carol J. Baker, MD, researchers
are working to develop a vaccine for RSV. In
the meantime, she and other doctors advise
parents to practice healthy habits like frequent
hand-washing and to seek medical attention
if baby develops any of these serious symptoms:

 Thick nasal discharge
or a cough with yellow,
green or gray mucus
 Difficulty feeding
 Signs of dehydration

Do I really have to take


my babys coat off when
hes riding in his car seat?
A: The short answer is: yes! Why is it

1 Heat up the car ahead of time if it


is safe to do so, so its already warm
when you get in.
2 Wrap baby up in a blanket as you
transport him to the car. Once youve
secured the harness correctly, place
the blanket over baby, and tuck it in
on either side of his body.
3 Heat easily escapes from the head
and feet. Keeping these areas of the
body warm will help regulate body
temperature as you transport baby
out of the house and into the car. A
cozy hat and thick socks or booties
will provide the extra warmth your
little one needs.
4 Many infant carrier car seats come
with a warming boot to put over the
baby. If not, one may be purchased
from the car seat manufacturer or
from another approved manufacturer.

What should I do at the


first sign of diaper rash
on my infants bottom?
A: Diaper rashes are common, but they can be one of the most frustrating problems to
treat in a pediatricians office. Thats because they are often caused by a combination of
factors, so fixing one problem at a time doesnt always do the trick.
At the first sign of diaper rash, soak your baby in a warm bath of clear water (no soap).
Gently clean the diaper area using a soft cloth or your hand.
After the bath, dry your baby gently, but do not replace the diaper right away. Allow
her skin to become fully dry before you use any creams or ointments. This is the most
common mistake parents make when treating diaper rash.
Once your babys skin is completely dry, cover it generously with a water-repelling
barrier cream. Most over-the-counter creams are water-repelling, and they dont need
to be expensive. Many brand-name products also contain anti-inflammatories, antifungals or vitamins to encourage skin healing. I tell parents to apply the cream like
spreading icing on a cake. In other words, go thick! There is no danger of overdoing it.
After that, watch your baby carefully. Change her diaper immediately when it is wet
or soiled. Although you may see some improvement right away, it can take several days
to heal entirely.
A new diaper rash is often triggered by chemical irritation. Chemicals can be artificial,
such as those in baby wipes, or natural, like urine. Disposable diapers contain chemicals
that may irritate babies skin, and detergent residue in cloth diapers can be a factor, too.
Chemical irritation leads to inflammation, and inflammation can persist long after the
chemical is removed. In these cases, a mild anti-inflammatory cream, such as 1 percent
hydrocortisone, may be helpful in older babies.
Microorganisms play a role as well. Yeast is common in our intestinal tracts and is in
poop. It grows well on warm, moist skin. For that reason, an antifungal cream might also
be helpful for difficult rashes.
Babies have sensitive skin to begin with, and diapers make the problem worse by
keeping the area humid. Allowing your baby to have some diaper-free time is often the
best trick to healing a stubborn rash. If that doesnt work, its time to talk to your
pediatrician.

uncomfortable he would be strapped
in while wearing a heavy coat for
the duration of the ride. His body
temperature may actually heat up too
much with all the thick material and
lack of airflow.)

Both Owens and Perez attest to the
name-changing process being a simple one.
I went to the courthouse, filled out a form,
paid a fee, and now its being published
for four weeks in the local newspaper.
Then well have a hearing, and after that,
its done! says Owens. I was thinking it
would be hard and maybe not even worth
it, but its been so easy.
Rosenkrantzs advice: If youre going
to make a legal change, do it as soon as
possiblebefore you and the child get too
used to it. Its much easier for a newborn
to adjust to a new name than a 4-year-old.
The procedure varies from state to state,
but your first step is always to call or visit
your local county court to find out exactly
what you need to do to make the change
happen. (Expect a fee, although its
typically a modest one.)
The hardest part of the process might
just be the emotional aspect. I felt I failed
my baby, admits Perez.
I was so scared to tell people, says
Owens. She also questioned whether a
change was what she really wanted. I
thought, Oh, its just the hormones. Im
sleep-deprived, Owens recalls. But when
the feelings of remorse didnt go away,
she knew shed made a mistake. And so
Wylder June became Lyla June, which
everyone agrees is a better fit.
To help you settle on a name that you
wont end up wishing to change, Owens
offers up some advice: Say it out loud,
all day every day. Let it roll around in
your mouth and in public and hear other
people say it. Practice with it! It was not
