Message from the Customer Service Manager


Well, its time once again for giving thanks. Thank you all your hard work
during our very extended busy season this year. Thank you to all of our new
facilities for helping to expand our acceptance facility network.
Thank you to all the facilities that organized passport fairs as a service to
their communities.
Thank you for your flexibility and patience in dealing with Passport Services
newest requirement for no eyeglasses in photos, and all the other changes
that weve requested of you in the new PARG.
Thank you for the 2017 annual certifications that are already trickling in.
Thank you to all of our new facilities for helping to expand our acceptance
facility network.
Thank you for the excellent knowledge and information security controls
you displayed that gave CPCs region an overall compliance score of over
95%. (The 20+ perfect scoring facilities from the latter half of the year will
receive their own thanks and recognition next month!)

Some applicants may have questions about taking their service animal abroad. The Department
of State does not issue passports for animals. According to travel.state.gov, a traveler with a
service animal should contact the U.S. embassy or consulate of their destination country for
information on restrictions and cultural norms about service animals. They should find out
about any quarantine, vaccination, and documentation requirements.

Passport Services may issue a second limited-validity passport book in


cases where:
A foreign country may deny entry or a visa to the bearer of a passport containing markings
or visas showing travel to certain other countries, OR
When a passport needed for immediate travel is unavailable due to prolonged visa processing delays.
An application for a second passport is executed and processed in the same manner as other
passport applications. The applicant must submit the passport fees, photographs, and evidence
of U.S. citizenship and identity, but must also provide a signed written statement with the reason(s) why a second passport is needed and that if lost, the loss of either passport will be reported to Passport Services or the nearest U.S. Embassy or Consulate immediately.
The second valid passport will be limited in validity to only two years and may not be extended, even though the full passport fee is paid. An examplar of the written statement is available
on Document Library section of the PARG.

If a mother or father under the age of 18 applies on behalf of his/her child, the minor parent
will need to submit one of the following:
Proof of emancipation
Proof of marriage
Proof of service in the U.S. Armed forces
If none of the above are available, the minor parent must submit:
Written statement of consent from his/her parent/guardian (the grandparent of the applicant) stating the minor parent may apply for a passport on behalf of the child
Evidence of relationship between the parent and grandparent (birth certificate)
If the name of the grandparent on this document does not match the name on the
birth certificate, please also submit evidence of a legal name change
A clear photocopy (front and back) of the grandparents identification

An applicants signature does NOT have to be legible to be acceptable. Please do


not advise applicants to print their name on the signature line. They should use their
natural signature.
In some states, when someone applies for a new drivers license, the DMV will give
them a temporary paper copy and send the actual card in the mail. If someone
comes to your facility to apply with that paper copy, they must also submit a copy of
their old expired drivers license. Even then, it might not be enough.You should tell
the applicant that it is up to the passport specialists discretion on whether it will be
accepted or not. Passport Services may request that the applicant submit a copy of
the actual card when it arrives.
When accepting a no-fee application that includes Form DD 1056, you are not to
write on any portion of the form. The Agent Info section refers to the military
agent handling the applicants travel orders.

Your 2017 Annual Certification paperwork should have already arrived. All Passport Acceptance Facilities are
required to submit an Annual Certification Packet in order to remain in the Passport Application Acceptance
Program. The completed packet must be received by the Customer Service Manager at your regional passport
agency by January 31, 2017. Annual Certification packets received after this date will not be accepted. Any facility
that does not submit a completed packet by January 31, 2017 will be suspended from the Passport Acceptance
Program and cannot accept passport applications or offer passport services until further notice.

Things to Remember When Completing the Packet:


The information provided must be complete and accurate.
The information provided must be legible and typed or written in black ink.
Signatures and initials must be present where requested, even when submitted electronically.
The paperwork must include proof that all agents have successfully completed Webbased or In-Person Acceptance Agent Training between the dates of January 1, 2016
and December 31, 2016.
All agents must complete training in order to continue accepting applications in 2017.
Those who do not complete training will be removed as passport acceptance agents.

Assuming the application is complete and issuable, customers will


receive their passport books/cards approximately 4 to 5 weeks from
the date of application. Those applicants who wish to receive
passports sooner may request expedited service and two-way
overnight mail to receive passports within 2 to 3 weeks (Additional
cost per application is $60.00 plus two-way overnight mail). For applicants with proof of more urgent departure (within 2 weeks), applying in person at a Passport Agency is recommended.
