Subscription Certificate

I confirm that I have read and understood the PLDT Terms & Conditions concerning my subscription to Home DSL,
including any amendments found at www.pldthome.com.
I understand that my subscription is for a minimum of twenty four (24) months from date of sign up for DSL Plan,
subject to the availment of special promos which may require longer subscription periods (the Minimum Term). By
signing below I am eligible for freebies and promos discussed with me upon subscription.
After the Minimum Term, my subscription will be automatically renewed for the same period, terms and conditions,
unless I submit a written request for cancellation to PLDT prior to the end of the Minimum Term. PLDT reserves the
right to change the plan offers after the Minimum Term. I agree to pay downgrade or pre-termination fees as set by
the current PLDT policy--equivalent to three (3) times the plans Monthly Service Fee (MSF) should I opt to
downgrade my subscription plan or terminate the same during the Minimum Term or renewal thereof. I further
understand that this remedy shall be in addition to such other remedies to which PLDT is entitled to under the law,
including but not limited to remedies granted under Article 315 of the revised Penal Code.
In regards to selecting the DSL Speedster Plan 999:
I understand that (1) there is a corresponding speed and volume allocation for my subscription plan; (2) upon
reaching the optimum monthly volume allocation for my account, I have the option to purchase additional volume
allowance via Booster Package.
With respect to the ownership of the WiFi modem:
I understand that upon full payment of the WiFi modem, I shall own the same and I shall have no obligation to return
the same to PLDT upon the termination of my HOME DSL subscription. PLDTs responsibility for any damage to the
WiFi modem shall limited to repair of the defects thereon within the 12-month warranty period. PLDT shall not be
liable for any damage to, or loss of, the WiFi modem due to my fault or negligence, or due to force majeure.
In the event I pay the WiFi modem in installment, I understand that until I complete the installment payments for the
WiFi modem, PLDT shall continue to own the WiFi modem and I am obligated to use and take care of the same with
due care. Should the WiFi modem be defective or destroyed without my fault or negligence, upon my request, PLDT
shall replace such WiFi modem at no additional cost to me. However, should the WiFi modem become defective or
destroyed due to my fault or negligence, I shall be obligated to pay PLDT for the WiFi modem before PLDT provides
a replacement therefor. Should I pre-terminate my subscription to HOME DSL before the end of the Initial Term, I
shall be liable to pay for the balance of the WiFi modem fee.
Upon my completion of the installment payments for the WiFi modem, I shall become the owner thereof and I shall
have no obligation to return the same to PLDT upon the termination of my HOME DSL subscription.
