Independent Study Mentorship- Fall 2016

What is Independent Study

Mentorship? (ISM)
Independent Study Mentorship is a collegiate level course for students who are
wanting to have hands on experience in a field of work. The students in the program
have a mentor for the course of a semester. They shadow them at their work place to
gain information about their selected topic and other information the student learns.
Within the class we have multiple assignments to help us get to our final project, and
things that allow us to better understand our selected profession

My Mentor
My mentor is Valerie Gray
She has her Bachelors in Nursing
She graduated from UTMB Nursing School in Galveston in 1993

She currently works at Clear Lake Regional

Why this topic?

I choose the topic of feeding because its something that is vital towards all stages of
life. Its also very underestimated the amount of work and effort that goes into feeding a
baby, even more so with a baby in the NICU.

Common equipment and schedule

Bottle and syringe warmer, external feeding , and a picture of the board with the
times to feed and what to feed.

The top row is items commonly used for NG tube feeding

and the bottom row is commonly used for bottle feedings

Feeding Tubes
While in the NICU most babies are trying to still grow due to being premature , and also fighting
of diseases. Its common that these are done though the nose or the mouth straight to the
stomach. Tubes are commonly placed using an X-ray machine for correct placement , either
in the stomach for normal feedings or small intestine for slower feedings.

Open needed equipment

Fill syringe with amount that the baby
needs

Connect external feeding tube with the

feeding tube that inserted into the baby
Slightly push down the syringe and fill the
tube with liquid till it comes to the other
end

Connect syringe with external feeding tube

Once syringe is warmed connect to feeding

tubes, after connected hang syringe in a
dispenser and set for the amount of time to
administer food, once all finished
disconnect external tube from internal tube
and throw way syringe, and tube.

Tube Feedings
Tube feedings are done with a syringe filled with either breast milk or formula ( in ccs)
then connect to a tube, like the ones in the picture below, which will connect to
another tube that is comes out of the babies mouth or nose.

Complications of Feeding Tubes

It normal for babies for develop a nasal infection or irritation of the nose, mouth or
stomach. Minimal bleeding can occur within the stomach wall because of irritation
from the tube. The tubes may also lower the babies heart rate, breathing rate and
cause trouble if the baby tries to spit up.

Types of Bottles

The bottle above is

commonly used when a
baby starts on bottle
feeds at 32 weeks or
once they are healthy
enough or strong
enough to start rooting.

The picture above shows different nipples that

are used. Each baby takes to different types of
nipples so the hospital has a variety of types that
are used. Each one is a different size and/or
shape.

Process of getting bottles ready

Pour formula into the bottle

Open the bottle, formula and nipple
Measure out the amount that
the baby needs

Warm bottle than feed it to the baby and

dispose of afterwards

Attach nipple to bottle by using the clear

protector so you dont directly touch the
nipple

Oral Feedings

A baby at normal gestational age for feeding is 32 weeks, this is the age that babies
start to have the rooting effect. The babies are rated on a scale called Early Feeding
Skill(efs) to see if they can start full bottle feeds. Bottle feeds creates a physiologically
and behaviorally challenges. They develop more motor skills and more alertness.
Starting bottle feeds is a positive movement toward getting discharged, and becoming
stronger and healthier. The nurses may allow the parents to start kangarooing for the
baby to get used to the smell of the parents and later start feeding them.

Complications of Bottle Feedings

While bottle feedings the baby may get tired and become distressed, this s commonly
prevented by only letting the baby eat fro 30 minutes. The baby may also have less
breaths due to sucking in milk. The oxygenation is most common to drop within the first
few minutes while the baby gets used to breathing and eating at the same time, this is
called apnea and can lead to hypoxemia. There may also be Negative
cardiorespiratory responses when the baby focuses on the eating aspect more then the
heart function.

Conclusion
Within this presentation we have talked about neonatal intensive care unit tube
feeding as well as bottle feedings. We also discussed the complications of both
feedings and looked at different aspects of them.

Thank You!
I want to thank my mom who has helped me and encouraged me to continue going ,
my dad who has been a rock when I thought it was all to much and wanted to give
up and my brother who made me laugh when I was having a rough day.
I also want to thank my mentor, Valerie Gray, who has so willingly allowed me to
shadow her this semester, and answered all my random questions , and helped me
learning more about the Neonatal Intensive Care Unit.
I want to thank my cousins Bry and Beau who allowed me to tape me feeding
EmmyLou for tangible item.

Lastly, I want to thank Mrs. Click, my ISM teacher, who has given me the opportunity to
be in the field of work that I want to work in after college and has given me the tools
and work to make me successful for this year.
