Directions for results: Count up your total for each wellness category and
read the appropriate scoring category below.
Scores of 16-20: Your answers demonstrate that youre already taking positive
steps in this dimension of wellness. Although you achieved a high overall score in
this domain, you may want
to check for low scores on individual items to see if there are specific areas you
might want to address.
Scores of 11-15: Your behaviors in this area are good, but there is room for
improvement. Take a look at the items on which you scored lower. What changes
might you make it improve your score? Even a small change in behavior can help
you achieve better health and well-being.
Scores of 10 and below: Your answers indicate some potential health and wellbeing risks. It is probably time to make a change. Now would be a great time to
develop and set achievable goals to help you get where you want to be.
Explanation of Assessment
After using Google to search through several health, fitness and wellness
assessments, I came across one assessment that I felt was concise and that
covered several aspects of overall wellness while focusing questions on specific
areas of wellness. I essentially took questions from the assessment that I felt related
appropriately to my workplace, Starbucks. I tweaked some of the questions in order
to relate them appropriately as well as eliminated some questions that did not make
sense for my workplace. The original assessment can be found at:

I enjoyed the form of this assessment because of the way the scoring was
completed. I liked being given four options to choose from for an answer as opposed
to yes or no so that there is a more appropriate and accurate measure of wellness in
each of the categories. Also, I liked the format of this assessment because it
focused on multiple wellness categories as opposed to just one. This is just one

short assessment, but if scores are very low in some wellness categories, individuals
can take it upon themselves to seek out other assessments in the specific wellness
categories they scored low on and would be able to develop and set goals in those
specific wellness areas that will improve their wellness in those areas.

Reflection
After completing this health and wellness survey about my current job and
workplace, which is Starbucks, I can see a pattern and definitely need to make some
changes in some of the areas in order to improve my overall wellness. I need to find
more pleasure within my occupational wellness. I scored a 12 in this category,
showing me that I am okay where I am, but over time I may need to change
occupations in order to remain satisfied. This works for me because I intend to leave
Starbucks eventually to pursue something that I enjoy more, which is working with
kids.
Areas that I feel that I am succeeding in include my environmental and
intellectual wellness. I think that my intellectual wellness is so strong because I
enjoy learning new things and I always want to be learning and understanding how
to do my job better. I believe that this links with my high score for environmental
wellness as well, as I work hard in both areas to be successful at my job which in
turn makes me a positive asset in the work environment. The lowest rating, I
received was a 12 in the occupational category. I do believe this is a true state of
my occupational wellness as it is not my dream job and so I am not fully strong in

that state currently. I scored in the middle in the physical and social wellness areas,
getting a 15. I think that score is indicative of my true state of wellness in both of
those areas. I am currently busy with working 32 hours a week and taking 3 3-credit
college courses online. I have very little time or energy for physical activity other
than being on my feet at work for 8- hours at a time 4 days a week.
I could use this information to lead my workplace towards a more healthy and
active lifestyle by organizing a day or two out of the week where my co-workers and
I get together to workout. Also, I could encourage co-workers to participate in a
healthy eating challenge and create a prize for the winner each month. Lastly, I
could encourage my boss to organize a monthly meeting where we can all meet and
discuss ways our communication as a team can be heightened as well as get a feel
of how everyone feels about the environmental wellness and social wellness. This
could help boots everyones wellness in those two categories.
