( Arrival )

Boarding Agent shall collect original ship certificates

and Passport for Clearance purposes

Preparing all doc and apply form to

Harbour Master
1. Phyically check Original ship certifcates
2. Registration administration at Harbour Master office
3. Original Last Port Clearance
4. Original Last Port Immigration Clearance

a). Original Ship certificate

b). Arrival Crew list
c). Cargo manifest
d). Ship Inventory list
e). Customs Documentation ( Import / Export )
( for Custom Clearence ) if any cargo

Ensure all the certificates is still valid

f). ILR & FE Certificate should be valid

g). PKKA PERMIT
h). Original Last Port Clearance
i). Original last port Immigration Clearance
j). PSC Report ( Port State Control ) for vessel
more than > 500 GRT
k). Original Passport

than 6 month since date of expired

Ensure all the certificates is still valid

- Multiple Re-Entry permit ( MRP )

Visa A211 is required in Indonesia

Crew has Dahsuskim holder is not

required to apply EPO, due to his have
been holding Multiple Re-entry Permit
( MRP ), he can leave anytime.

3. Customs Seal Report ( Arrival )
4. Customs Open Seal Report ( Upon received onboard )
as per dsignated vessel to deliver next Port Call /

6. Letter of Authorization ( issued by Shipping
agent ) for Customs puroses only
