AFFIDAVIT
I, JOCELYN NABOA y ANDRES, of legal age, Filipino citizen,
married, and a resident of Barangay Palimbo Proper, Camiling, Tarlac,
Philippines, after having been duly sworn to in accordance with law, do
hereby depose and state THAT:
1. I finished my secondary education at the Macarang National
High School, Mangatarem, Pangsinan in the year 2001;
2. While some schools issue a yearbook to their graduating
students, the school where I graduated did and does not issue
yearbook;
3. I am executing this affidavit to attest to the fact that no
yearbook was issued to me by the said school;
4. Finally, I am executing this affidavit to attest to the truth of
the foregoing statements and for whatever legal intents and
purposes this may serve.
