CHAPTER I
Introduction
In this chapter, the writer would like to express about background, research
question, objective of writing and significance of writing.

1.1 Background
Every woman wants to have a pretty face, clean, and charming. A healthy facial skin
is identical with the smooth facial skin and free of skin problems such as acne, oily face,
and dull. Complaints are often experienced by most people at his face is acne. Acne is
one of the troubling skin disease especially in women.

It can be said that no one

in his life completely free from acne. Acne is common in adolescents, ages 15-19 years
in women and 17 21 years old guy, but sometimes occurs in children and adult women
in menstrual period. The emergence of pimples often occurs during puberty caused by
hormonal changes. Acne can also be influenced by a person's age, genetic factors and
less facial cleanliness.
Although more appear in teenagers, but that doesnt mean the breakouts were not
adults, the results showed as many as 85% of the population experiencing acne at the
age of 12 to 25 years, 15% of the population are experiencing up to the age of 25. If it is
not resolved it can be settled until the age of 40. Although this problem is considered
mild and treatable himself, but it considered a disease of concern because it can reduce
the confidence in particular adolescents.
The skin becomes less beautiful because it exposed to problems such as scars, spots
and acne scars. Acne also raises the main effect on the soul of a person, such as a crisis
of confidence and depression. Simple things that can be done to maintain the cleanliness
of the face occurs with cleaning the face using face soaps or facial cleanser so that
spared from other skin problems and also consume foods that are nutritious.
Along with the progress of the times, a wide variety of acne treatment is offered in
the community, with a range of advantages of each. But of course not all products
guaranteed security. A wide variety of chemical substances contained in the acne
medications thus probably will make acne worse and increased facial skin become less
beautiful.
Based on the above reasons then sought other alternatives to treat acne by utilizing
materials from nature. From the large number of natural ingredients available, one of
the ways that can be used in facial skin care is the use of Binahong leaves. Basically the
leaves of binahong also helps to address a wide range of illnesses even in wound
healing.
The use of natural materials in the prevention of the problem of facial skin is
better used than on chemicals, especially for teenagers. This is because teenagers have

skin that is more sensitive and vulnerable to a variety of skin problems, such as acne or
dull skin. In addition to the abundance of outdoor activities that can eliminate the skin's
natural beauty.
Base on this conditions , the writer is interested in researching more about
Anredera cordifolia dependencies with the treatment and prevention of skin problems
experienced by teenagers.

because there is some content that can make it as one of the herbal and natural treatment
without causing a negative impact on the body.
The plant comes from the latin , "Bassela rubralinn", in the United Kingdom,
"Heartleaf Maderavine", and in Chinese: Deng san chi.
Binahong (Anredera cordifolia (Ten.) Steenis) is a plant that is allegedly derived
from Australia, South Africa, Hawaii, New Zealand and other Pacific Islands. The plant
is easily grown in the lowlands or Highlands.
The plant, which is said to come from Korea is consumption by the people of
Vietnam during the war against the United States in the 1950s to 1970s. This plant is
also known among the Chinese community by the name of Dheng San Chi and have
had thousands of years consumed by the people of China, Korea, Taiwan, etc. Parts of
the leaves of this plant are usually used as a natural remedy other than rod.

C. Content of Anredera Cordifolia

Based on research results, Anredera Cordifolia contain saponins, alkaloids, essential
oil, Oleanolik Acid and polyphenols.

1. Saponins
Saponin is a sterol glycosides triterpena and has been detected in more than 90
tribes plants. Saponin is a surface active compounds and like a soap, as well as such can
be detected based on the ability to form foam and menghemolisis blood cells. A search
of saponins in plants has been stimulated by the need for a source of sapogenin easily
obtained. Saponin and sapogenin glycosides is one widespread type of glycosides in
plants. There are two kinds of triterpenoid saponin glycosides, namely alcohol and
glycosides with steroid structure.

2. Polyphenols
Phenol compounds is diverse compounds derived from plants, which have the
same characteristics, namely aromatic ring containing one or two hydroxyl penyulih.
Phenolic compounds are more soluble in water because it is often bound to sugar as
glycosides, and usually found in the cell vacuole. some compounds phenolic have
known structure. Flavonoid the largest group is, but a simple Monocyclic phenol,
phenyl propanoid phenolic Quinones, and there are also large in number. Some of the
important polymer materials in plants such as lignin, melanin, and the tannins are
polyphenols compounds.
3. Alkaloids
The alkaloid is the largest secondary plant substances. In General is alkaline
compounds includes alkaloids containing one or more nitrogen atoms, usually as part of
a cyclic system of alkaloids which are often poisonous to humans and many have
prominent Physiology activity, so is widely used in medicine.
4. Essential oil

Essential oils can originate at any part of the plant from a leaf, flower, seed, stem or
bark and roots or rhizoma. Essential oil eteris oil that is also known as a volatile and oil
obtained from plants by means of distillation, colorless especially when still in the fresh
State.
Essential oils are generally composed of a mixture of various chemical compounds
formed from elements carbon (C), hydrogen (H) and oxygen (O) as well as various
chemical compounds containing the element Nitrogen (N) and Sulphur (S). Some
essential oils can be used as material for internal and external antiseptic, analgesic,
hemolytic or enzymatic, sedativ, stimulant, a drug for stomach pain, cosmetic fragrances
and soap.
5. Oleanolik Acid
Binahong leaves are known to have acid oleanolik. Oleanolik acid is the
triterpenoid which is the antioxidants in plants. The mechanism of protection by acid
oleanolik is to prevent the entry of toxins into the cells and improve the defense system
of the cell. oleanolik Acid also has anti inflammatory substance.
Content of nitric oxide in oleanolik acid also being anti oxidant, can serve as a
powerful toxin to kill bacteria. So with the presence of oleanik acid it will strengthen the
resistance of cells to infection and improve the cell so that the cell can regenerate
properly.

D. Benefit of Anredera Cordifolia

The use of binahong leaves good for addressing health problems and also
beauty. Binahong store leaves a lot of benefits for all kinds of diseases in maintaining
health. The use of binahong leaves for health traditional medicine as indeed has
extraordinary potential to help treat a variety of health disorders. The important content
in the leaves of binahong this one is a saponin which is the distribution of compounds
from herbs have extraordinary potential to help treat various chronic diseases.

The content that is in the binahong bulbs contain triterpenoid saponins,

glycosides, steroids, and alkaloids. In leaves and stem is also found on the above
compounds that can be used and used for traditional medicine. Compounds that are
present in the leaf binahong above is useful to help treat the disease diabetes mellitus,
liver disease or hepatitis, cardiovascular disorders for example due to blood pressure,
high blood cholesterol, treat wounds. blood clotting, and stress.
Besides being used for body health, binahong is also beneficial for the care and
beauty of the skin such as acne and also can address to make your facial pores become
smaller.

2.2 Acne
Acne is a skin disease that occurs due to inflammation of the pores. The
inflammation is caused by dirt that clog oil ducts and then rocked the bacteria that cause
infection.
In the medical world, acne known as Acne Vulgaris, is a disorder or disease that
involves the oil glands in the hair follicle. This disorder typically emerges in
adolescence where that time when oil production is likely to increase.
Acne comes from the word "Acme", which means "high point", Come from Yunani
that has almost the same sense with dots or spots. Acne vulgaris is defined as a chronic
inflammation of the follicle polisebasea caused by several factors.
Acne vulgaris is a follicular disorder General about Sebaceous follicles (hair
follicles) are vulnerable and most often found in areas of the face, neck and upper body.
15% of teenagers have clinical acne (major acne) and 85% acne Physiology (minor
acne), which is the only acne consists of some blackheads only. Acne seems to stem
from the interaction of genetic, hormonal factors and bacterial. In most cases there is a
history in the family of akne. With the emergence of acne in adolescence, then an

awareness of the importance of the appearance of the self in social life that ultimately
could affect the self concept of young women.
Keep in mind that human beings have pores that are directly connected with the
oil glands that lie just below the surface of the skin. These glands are connected by
pores through the small canals, these glands produce an oily fluids or commonly called
sebum.
The function of this oily liquid is bringing the dead skin cells to the surface of
the skin to make it more easily cleaned and removed. However, when the small canal or
follicle undergoes a blockage of liquid oily clotting then under the skin's surface can not
be inevitable.

At the time of clogged pores and with the abundance of sebum on the skin's
surface, called Propionobacterium acnes bacteria quickly. This bacteria is a bacteria
causes acne. Propionobacterium acnes releasing some kind of irritant substances that
will irritate the skin. The effect of this is to cause irritation to the skin to Redden and
swell, beside that also cause the onset of pus beneath the skin. Layers in hair follicles
also thicken due to changes in hormone levels and cause blockage of the pores of the
skin. Blockage of the pores will not be lost, though the skin has cleared up.
If this condition continues then the end result is the emergence of acne. From
understanding the Acne can we understand that acne happens when disruption occurs a
blockage on the Canal or the follicles under the skin's surface.

A. Causes of acne
Causes of acne are very much is multifactorial, among other :
1. Hormonal

10

Everyone is going through the hormonal cycle turnover especially in adolescence,

known as puberty. At this time a hormone-producing gland could lead to excess oil.
Hormonal cycles also occur in a woman's menstrual period, one of which led to the
emergence of acne. The secretion of the sebaseus gland hyperactivity spurred by the
formation of the hormone testoterone (androgen) excess, so at the age of puberty will
arise a lot of acne on the face, chest, back, whereas in women other than androgen
hormones, the production of sebaseus gland lipida spurred by luteinizing hormone that
increases the time leading up to menstruation.
2. Genetic
Causes of acne due to genetic or congenital from parents. So if both parents of the
father or of the mother is a person who is easily affected by acne, then this possibility
could also be dropped onto his son.
3. Dead skin cells
As you get older, it will be going on turnover of cells in the body. Is no exception
with the skin. The dead cells will be replaced with new cells. But if not painstaking in
cleaning and caring for, then these cells will accumulate and mix with excessive oil
finally result in irritation and cause acne.
4. Bacterial infection
Bacteria reside in the skin called the p.acne, which tend to proliferate in the blocked
sebaceous glands, which produce substances that cause irritation to the surrounding
area. The glands continue to swell, and it may be broken, then spread the inflammation
to the skin area. This is what causes the onset of acne stone, i.e. leave a long-term
pigmentation and scars like smallpox that permanent. When dead skin cells and excess
oil which is infected by the bacteria then automatically also will also infect the skin.
Skin that's been infected because bacteria is what will experience irritation as a form of
body/skin reactions against the harmful bacteria from the outside.

11

5. Excessive oil production

Acne is a skin disorder due to abnormal conditions excessive production of oil
glands (sebaceus gland) which caused the blockage of the channel of hair follicles and
skin pores. The most common cause of acne is hormonal, piles of oil or sebum in the
skin is collaborating with bacteria. It is this circumstance which causes blockage in the
pores and hair follicles. This can be due to incorrect or incompatible with certain foods
6. Drugs
Consuming certain drugs could be one factor causes the onset of acne. Drugs that
contain certain chemical elements such as corticosteroids either drunk (oral) and that
their use could lead to a decline of dioles durability body. Decreased body durability
then the pathogenic bacteria activity will increase. Corticosteroid drugs consumption,
good oral (drinking cure) and topical (medicine oles), resulting in decreased body
endurance, also increases the potential incidence of acne due to the activity of the
pathogenic bacteria are increasing.
7. Stress
There are certain hormones that come out when someone is stressed, allowing the
growth of acne. Stress makes people have eating patterns that tend to consume a lot of
sugary foods and fatty foods. Androgen hormones hormone human is one of a number is
likely to rise in the age of adolescence. When androgens increases then oil production
also has increased.

B. Types of Acne
1. Blackheads.
Blackhead is the scientific name of a clogged pores, could be open or closed. Open
comedones, also known as blackhead, looks like an enlarged pores and blackened

12

caused penyumbat pore yg changes color due to oxidized by air. Closed comedones or
whitehead, have
skin that grows over the pores are clogged, look like small white bumps under the
skin. This type of acne blackheads are caused by dead skin cells and excessive oil
glands on the skin.
2. Regular Acne.
This type of acne is easily known, small pink bumps or redness. Occurs because the
pores are clogged with infected by the bacterium propionibacterium acne types. These
bacteria usually live in the channel blocked sebaceous glands, namely in the area where
in which the fatty acids on the SAC of the sebaceous glands are hidden inside the pores
of the skin.
Named propionibacterium because capable of producing propionik acid (propionic
acid). This is a type of anaerobic bacteria that can live without oxygen, and it took a
discrete aerotolerant which caused irritation to the surrounding area. Bacteria that infect
the can from the washcloth, makeup brush, fingers, the phone also. Stress, hormones
and the humid air, can enlarge the possibilities of the formation of acne.
3. Acne (Cystic acne).
Cystic acne is a bigger acne , great with inflammation, gathered around the face.
Sufferers of cystic acne usually also have close relatives who are suffering from this
type of acne. Genetically the sufferer has:
a. The over active sebaceous glands that flood the pores with oil glands
b. The growth of skin cells that are not normal that cannot regenerate as quickly
as normal skin
c. Has the response to excessive inflammation so that scarring on the skin

C. Symptoms and Characteristics

13

The symptoms of acne itself can be varied depending on the type and severity of
acne. The most common symptom of i.e. incidence of colored reddish bumps or pus
containing yellow. The characteristics of Acne can also be grouped into two categories,
namely acne inflamed and inflamed and not inflamed.
1. Inflamed Acne
Acne inflamed reddish colored characteristics and the inflammation and swelling,
also there is irritation of the skin. Although acne is not a serious medical condition, but
it's good you aware so as not to cause a more severe condition that ended in a fatal skin
damage. Therefore, it is important for you to promptly consult a doctor or health
professionals who understand the health issues.
2. Acne is not inflamed
This type of acne less noticeable are visible, but the cause of facial skin feels rough.
Although not visible, should immediately be addressed because it could become
inflamed acne.

D. Treatment of acne
Facial skin care routine is the most simple facial cleanliness and consume foods that
are nutritious to regenerate damaged skin tissue. Skin care can also be assisted with the
use of natural ingredients in skin care of the face. Acne treatment can be done by giving
topical medications, systemic medications, surgery, or a combination of these methods.
1. Topical Treatment.
Topical treatment is done to prevent formation of blackheads, suppressing
inflammation and accelerates healing of the lesions. Topical retinoids as anti
inflammatory comedolytic and widely used, such as adapalene, tazarotene, and

14

tretinoin. Benzoyl peroxide as an bakterisidal is also often combined with antibiotics.

Topical medications consist of:
1. Material irritant that can peel off the skin (peeling), e.g. sulfur (4-8%),
resorcinol (1-5%), salicylic acid (2-5%), and others. Drug side effects can be
reduced with the use of irritant that started from a low concentration.
2. Topical Antibiotics can reduce the number of microbes in the follicles for
example 1% tetracycline, erythromycin, damisin phosphate 1% 1%. Antiinflammatory topical corticosteroid creams, ointments or mild or moderate
strength, for example nikotinamide 4% other, zinc, for example ethyl lactate
inhibits the growth of 10% remains miniscule.

2. Systemic Treatment
Systemic treatment is aimed primarily to suppress activities remains miniscule
addition can also reduce inflammation, suppressing the production of sebum, and affects
the hormonal balance. Systemic antibiotics such as tetracycline, erythromycin,
doxycycline, and effective trimetroprim against p. acnes.
Hormonal drugs to suppress androgen production and competitively target organ
receptors occupied in sebaceous glands, such as estrogen or antiandrogen siproteron
acetate. This treatment is aimed at adult women sufferers failed with other treatments.
Systemic corticosteroids such as prednisone and dexamethasone given to suppress
inflammation and suppresses the secretion of the adrenal glands. Oral retinoid
isotretinoin or its derivation as to inhibit the production of sebum.
3. Skin Surgery
The need for action to repair skin surgical scar tissue due to heavy akne vulgaris
with inflammation, either hipertropik or hipotropik. The surgical action tailored to the
sorts of conditions and scarring that occurs. Type of action: surgery surgical scalpel,
surgical, chemical surgery, surgical frozen, and dermabrasion.
