Artifact Title: Parent/Student Information Sheet
Teaching Standard:

Description of the artifact and how it meets the standard and criteria:
I wanted to get to know my students in a variety of ways before they came into the
classroom. I asked parents to fill out a form that asked a variety of questions about their
child. It was important that I also give the students a chance to tell me about
themselves, so I gave them a sheet to fill out as well. It asked questions about their past
school years, as well as interests, hobbies, etc. From these sheets, I learned a lot about
the students and the parents, and I felt a lot more prepared for the first day of school.
These forms informed me of the life experiences that I needed to know of to have a
successful classroom. No two families are alike, and I wanted to give parents a
comfortable place to let me know of any information that I needed shared. It gave me an
opportunity to open the dialogue between parent and teacher, in the hopes that we
could grow together as a team for their child.
Reflection on the artifact:
I really enjoyed using these in my Back to School Night routine. I think that the parents
enjoyed it to because it gave them an outlet to talk about the strengths/needs of their
child, and any concerns that I needed to be informed on. I wanted to be prepared and
know what was needed from me, and I believe that these forms did a great job on that.
The forms told me what each child was good at, and what we needed to grow on, which
I believe is very important to know. There were a ton of papers and information that was

given to the parents that night, but I really think that they took their time on this form,
which was very meaningful to me because it showed me that they cared for their childs
education. I had many different parents take this form home and returned it to me on the
first day of school. I also believe that the students enjoyed filling these out as well
because they were able to share about themselves in their own words. It was nice to
hear a voice for each child before I really got to know them. These sheets also gave
me a lot of topics that I have used to interest the students in our daily lessons. I believe
that it is important to try and connect with each childs interests, so they feel that they
are an important part in our classroom. I will be using these forms again and again
because I believe it formed an open communication between parent, student and
teacher.
