AFFIDAVIT OF ABANDONMENT
I, SOLEDAD D. YAPUS, of legal age, Filipino, and resident of Block
6 Lot 9, Sta. Rita St., Pamplona 2, Las Pinas City, after having been duly
sworn to in accordance with law, hereby depose and say, - that:
I am the biological mother of the late SHIRLEY D. YAPUS, died
intestate on December 2, 2012 at the Philippine General Hospital, Taft
Ave., Manila;
She was a bonafide member of Social Security System with SSS
No. 34-1699300-9;
She was married to RAMIL G. EVANGELISTA, and they were blessed
with two (2) children namely:
1) ELY GABRIEL

That my late daughter when she was still alive and her two children

children above-mentioned.
I am executing this affidavit to attest to the truth of all the
foregoing facts and for all legal intents, effects, consequences and
purposes.
