Lifting Operation with Forklift Incident


SCOPE:
This ALERT is applicable to All Segments Using
Mechanical Lifting Equipment (WL, D&M, WS, WTT, CPL)

PURPOSE:
Control the Risk when conducting Mechanical Lifting
Operations with Forklifts and Cranes

WHAT HAPPENED:
While moving a long basket containing tools at about 2m height with a forklift , this one lost its hydraulic pressure then the
load fell on the ground. Hydraulic oil was then released from the forklift and created a spill. It could kill anyone would be
underneath or crossing the load.

Lesson Learnt:
Assign a Schlumberger Trained Employee to Supervise all Lifting Operations
Perform pre-inspection checklist for all Forklifts and Cranes before start of every Lift
Never put yourself underneath or cross the lifted load or the warning tape anytime.
Conduct pre-job meeting with All involved including driver operator
Clear Lifting Area and Itinerary and put a Warning Tape
Comply with Mechanical Lift Plan, Standard 13 and Angola Mechanical Lifting Process at all times
Forklifts to move equipment less than 30cm above the ground and minimum height with cranes
Only certified employees to be involved in Mechanical Lifting Operations.
