DEED OF DONATION

KNOW ALL MEN BY THESE PRESENTS:

That the DONEE is a cousin of the DONOR, who has lovingly dedicated five (5) years of his life
as the latter's personal caregiver and companion;
That FOR AND IN CONSIDERATION of the DONEE'S trust, devotion and affection shown to
the DONOR, and as an act of gratitude and liberality on his part, the DONOR hereby voluntarily
GIVES, TRANSFERS, and CONVEYS by way of donation, unto the said DONEE, his heirs and
assigns, the above described property, together with all the improvements found thereon, free
from all liens and encumbrances;
That the DONOR affirms that this donation is not made with intent to deceive his creditors, and
that he has reserved for himself sufficient funds and property;
That the DONEE hereby accepts and receives this donation made in his favor by the DONOR,
and hereby manifests his gratefulness for the latter's generosity.

WITH WAIVER AND RENUNCIATION OF RIGHTS
KNOW ALL MEN BY THESE PRESENTS:
WHEREAS, CECILIA LAUREL-SALAZAR, died intestate at Brgy. Poblacion 4,
Talisay, Batangas on February 13, 2000 leaving no will nor debts settled to date but leaving three
(3) parcels of land situated at Brgy. Miranda, Talisay, Batangas;
WHEREAS, at the time of her death, she left as forced heirs, the following and to wit:

JUANITO L. SALAZAR, married, of legal age, Filipino citizen and a resident of


Brgy. Poblacion 4, Talisay, Batangas;

MILAGROS L. SALAZAR, single, of legal age, Filipino citizen and a resident of


Brgy. Poblacion 4, Talisay, Batangas;

HERMOGENES L. SALAZAR, married to Ma. Teresa Lacanlale, of legal age,


Filipino citizen and a resident of Brgy. Poblacion 4, Talisay, Batangas;

WHEREAS, they are all her legal heirs by reason of the fact that they are all her
legitimate children and who are supposed to inherit by way of intestate succession;
WHEREAS, at the time of her death, she left three (3) parcels of land together with
improvement thereon and more particularly described and bounded as follows:
A parcel of residential land situated at Brgy. Miranda, Talisay, Batangas containing an
area of Four Hundred Twenty Nine (429) Square Meters, more or less; bounded on the North by
Willie Revillame; on the East by Bo. Road; on the South by Pedro Laurel; and on the West by
Coco Martin; covered by Tax Declaration No. 014-01724 of the Assessors Office of Talisay,
Batangas with an assessed value of P18,020.00
A parcel of agricultural land situated at Brgy. Miranda, Talisay, Batangas containing an
area of Three Thousand Five Hundred Seventy Two (3,572) Square Meters, more or less;
bounded on the North by Piolo Pascual; on the East by Sharon Cuneta; on the South by Benigno
Aquino; and on the West by Charo Santos; covered by Tax Declaration No. 014-02417 of the
Assessors Office of Talisay, Batangas with an assessed value of P16,780.00
A portion on a parcel of agricultural land situated at Brgy. Miranda, Talisay, Batangas
containing an area of One Thousand Five Hundred (1,500) Square Meters, more or less; bounded
on the North by Mitche Mae Rojas; on the East by Bo. Road; on the South by Camille Rojas; and

on the West by Bignay River; covered by Tax Declaration No. 013-02013 of the Assessors
Office of Talisay, Batangas with assessed value of P7,050.00
WHEREAS, in view of the above facts and circumstances, we are therefore co-owners
and to terminate co-ownership over the same, we hereby adjudicate over the same in pro-indiviso
sharing pursuant to Section 1, Rule 74 of the Rules of Court of the Philippines, subject to the
liabilities imposed by Section 4, of the same Rule;
WAIVER OF RIGHTS
WHEREAS, I, JUANITO L. SALAZAR (whose personal circumstances is mentioned
above) have agreed and covenanted to CEDE, TRANSFER and CONVEY unto my co-heirs
MILAGROS L. SALAZAR and HERMOGENES L. SALAZAR (whose personal circumstances
are mentioned above), their heirs and assigns the full ownership and possession of the abovedescribed properties.
NOW, THEREFORE, for and in consideration of the foregoing premises, I waived and
renounced, as by these presents I hereby WAIVE and RENOUNCE my share, right and
participation over the above-described properties.
That the above-described properties has not yet been registered under Act 496 nor under
the Spanish Mortgage Law and that the herein parties do hereby agree toregister this instrument
under the provision of Act 3344, as amended.

(Donation to the Government with Revocation Clause)


DEED OF DONATION
KNOW ALL MEN BY THESE PRESENTS:

This Deed of Donation made and executed by and between:


_____________, of legal age, Filipino, (single / married / widow), and a
resident of _____________, Philippines, (hereinafter referred to as the
"DONOR")
- and _____________ CITY, a municipal corporation existing under the laws of the
Republic of the Philippines, represented herein by its __Position__,
__Name of Official__ (hereinafter referred to as the "DONEE");
WITNESSETH:

WHEREAS, the DONOR is the true and registered owner of certain


playground equipment, listed and described in Annex "A" and attached
hereto and made an integral part hereof;

WHEREAS, the DONOR recognizes and supports the noble task of the DONEE
in
promoting
the
welfare
and
well-rounded
development of the
children of the City;

WHEREAS, the DONOR desires to assist the DONEE and give further impetus
to its numerous child welfare programs;
NOW, THEREFORE, for and in consideration of the foregoing premises and
due to the magnanimity of the DONOR, the latter hereby transfers and

conveys unto the DONEE, by way of DONATION, the playground equipment


listed in Annex "A" hereof subject to the conditions that the DONEE shall, at
its own expense, maintain the donated playground equipment and its
immediate surroundings in a clean and safe condition, re-paint the said
equipment on a regular basis to prevent corrosion and make all the
necessary maintenance and repair at all times to ensure that the playground
equipment remain clean and safe for use by the children. In addition, the
DONEE shall install and maintain on the premises signs acknowledging the
donation by the DONOR of the playground equipment.

In the event that the conditions of the donation, as contained herein, are not
complied with by the DONEE within thirty (30) days after the latter's failure
to comply with the same is brought to its attention, this donation shall be
automatically revoked and rendered ineffective and the DONOR shall regain
ownership over the playground equipment and shall have the right to
recover possession of the same.
The DONEE hereby accepts the donation of the playground equipment as
listed in Annex "A", together with the conditions herein imposed, and hereby
expresses its gratitude and appreciation for the support, kindness and
liberality of the DONOR.
