SITUATION: The client is a 47 yr B/F who was


hospitalized on 3/10/94 because she had a
Adiabetic reaction. The client is in room
402A, unit 4South MVH. Shes resting
quietly in bed after bathing in the bathroom
at the sink. Its 10:30 a.m.
Pre-interactional Phase - I went to the
clients room 30 minutes before the nurseclient interaction so that I would be able to
recognize her later and view the conditions
of the setting. The client was going into the
bathroom. We did not speak at this time. I
was placed at ease because of her pleasant
demeanor.

Introductory Phase
Nurse: AHello, I was told by the staff nurse
that you are Mrs. C.K. (Entered the clients

Nurse: My responsibility as a nursing
student is to communicate with clients and
encourage them to share their concerns
regarding the care and progress theyve
experienced since hospitalization. I would
like to talk with you for the next 20 to 30
minutes about how you feel concerning
these matters. (Touched the clients
shoulder gently, shared eye contact).

I identified the goals for the
interaction and set time
limits. The nonverbal

encourage further trust
acceptance of myself as a

the usual cue that a person is
willing to interact.

Interactional Phase
Client: I am learning a lot about how to care
for myself when I return home. I do not
want to faint and become ill again as I did
when I came to the hospital this time.
Nurse: Please continue, tell me more about
what youve learned.
Client: I now realize that I must eat the
foods required on my diet an that I must eat
food at the designated meal and snack
times.

Nurse: I assume you have not been doing


this in the past. Will you explain the reason
you have not been eating properly?

I wanted to know if the client


lived alone and was
disinterested in cooking or
eating alone, or if the reason
was due to lack of money.

Client: I eat alone and most of the time I


prefer not to cook. But this is going to
change now that I know who important it is
to eat the right foods at the right time.
Termination Phase
Nurse: Well Mrs. K., our time is nearly over
and weve both discovered some new and
