Sub: Request for issuing a new Report card.

Dear Madam,

I am guardian of Afia Zahraa, a student of KG-1 of your class (Morning Section, Jasmine). I
am very sorry to say that my childs report card is lost during the Summer and Eid ul Fitr
vacation while we changed our home.

This card was issued at the time of first term result and was due to return after vacation. But
I could not find it when the school opened after vacation. Therefore, I humbly request you to
issue a new report card so that I can track my childs learning progress throughout the year.

My child details are mentioned below:

Roll: 7

Section: Jasmine (Morning)

Session: January - December

I would like to request you to please process my request and issue a new report card at the
earliest. In case there are some charges for issuing the new report card then please inform
me.
