I would like to formally inform you that I am resigning from my role as a

Nurse in Leyte Provincial Hospital, effective June 01, 2016.

My tenure with Leyte Provincial Hospital has been a memorable experience

and the knowledge and skills acquired are priceless. Thank you for all the
advice and guidance provided by you and other members of the staff. What I
appreciate most is a working environment in which we are allowed to fail and
we are given opportunities to learn from our mistakes. It has been a positive
and invaluable experience working with all of the hospital staff.

Thank you for giving me the opportunity to serve and be part of your team.
Thank you for the support and the opportunities for professional and
personal development given to me during my time in the hospital.
