MEMORANDUM OF AGREEMENT

Know All Men By These Presents:

This Agreement entered into this 15th day of April 2016, at Malitbog, Bongabong, Oriental
Mindoro by and between:

INNOVATIVE COLLEGE OF SCIENCE & TECHNOLOGY, an educational institution duly

organized and existing under Philippine Laws with principal office at Malitbog, Bongabong, Oriental
Mindoro, herein represented by its President, MR. ANTHONY L. YAP, and hereinafter referred to as
ICST or the First Party.

LA CARMELA DE BORACAY an institution duly organized and existing under Philippine Laws
with principal office at Station 2, Balabag, Boracay Island, White Beach Path, Boracay herein
represented by its Human Resource Associate, MS. APRIL JEAN S. LEDESMA, and hereinafter
referred to as Industry Partner or the Second Party.

WITNESSETH: That-

WHEREAS, INNOVATIVE COLLEGEOF SCIENCE & TECHNOLOGY offers Bachelor of Science in

Hospitality Management intended to expose its students to the challenges of the hospitality industry
environment as well as provide business and academe the opportunity to collaborate in the education
of the future professionals;

WHEREAS, under the said program, junior or second year students of ICST, also referred to as
Trainee or Trainees, are assigned to sponsoring companies where they perform activities and tasks
along their lines of specializations for a period of Three Hundred Sixty Hours (360 hrs.).

WHEREAS, LA CARMELA DE BORACAY desires, and agrees to become a sponsoring company (or
Industry Partner) under such terms and conditions as hereafter agreed by the parties;

NOW THEREFORE, for and in consideration of the foregoing premises, the Parties agreed as follow:

1. Responsibilities of ICST
1.1 ICST shall request from the LA CARMELA DE BORACAY the accommodation of its Trainee/s
for the Internship Program.
1.2 ICST shall provide LA CARMELA DE BORACAY the list of qualified student trainees and the
requirements needed before the accommodation.
1.3 Upon advice from LA CARMELA DE BORACAY, ICST shall notify and request the student/s to
report to LA CARMELA DE BORACAY for interview.
1.4 Before the On-the-Job Training, ICST shall orient the Trainee/s on the compliance to the
prescribed work hours in the hotel/food service operation as well as to all the policies, rules and
regulations of the Industry Partner/Second Party.
1.5 ICST shall impose an appropriate disciplinary action to an intern as it may deem necessary in
case of breach or violation of the policies, rules and regulations of ICST and the Industry
Partner/Second Party.
1.6 ICST shall provide the LA CARMELA DE BORACAY a Performance Evaluation Form to monitor
and rate the Trainee/s performance.
1.7 For coordination purposes, ICST designates its Practicum Coordinator as the focal person on its
behalf, and who shall act as liaison between the partners.

2. Responsibilities of LA CARMELA DE BORACAY

MEMORANDUM OF AGREEMENT

2.1 Notifies the ICST of its action to the request.

2.2 Interviews/screens the applicant Trainee/s.
2.3 Assigns work, task or responsibility, as well as determination of work schedules.
2.4 Screens and supervises the Trainee/s vis--vis work assignment.
2.5 Monitors and rates periodically the Trainees work, behavior or attitude and performance through
the OJT Performance Evaluation Forms.
2.6 Grants subsidies, allowances or stipends to Trainees if possible, in accordance with and subject
to the companys policies, procedures and practices.
2.7 Submits the Performance Evaluation Report to ICST Practicum Coordinator or through courier.
2.8 Issues a Certificate of Completion upon completion of the required number of OJT hours.

This Agreement shall have a term of two (2) years, commencing from April 15, 2016 and may
be renewed under such terms and conditions as may be agreed upon by the parties. Each
Party, however, shall have the right to terminate this Agreement for any reason whatsoever,
upon prior written notice at least fifteen (15) days from termination date.

IN WITNESS WHEREOF, the Parties have signed this Agreement on the date and place first
above-mentioned.
