ABSTRACT
Antecedentes
La separacin de la madre y el neonato despus del nacimiento es comn en la cultura occidental. El contacto temprano piel a piel comienza idealmente en
el nacimiento e implica ubicar al recin nacido desnudo, con la espalda cubierta con una manta templada, en posicin decbito ventral, sobre el pecho desnudo
de la madre. Segn la neurociencia de los mamferos, el contacto ntimo inherente en su lugar (hbitat) evoca neurocomportamientos que aseguran el
cumplimiento de necesidades biolgicas bsicas. Este momento puede representar una "etapa sensible" desde el punto de vista psicolgico para la programacin
del comportamiento futuro.
Objetivos
Evaluar los efectos del contacto temprano piel a piel en el amamantamiento, comportamiento y adaptacin psicolgica de la dada madre-recin nacido.

Criterios De Seleccin
Estudios clnicos aleatorizados y cuasi aleatorizados que comparan el contacto temprano piel a piel con la atencin hospitalaria habitual.
Resultados Principales
Se incluyeron treinta estudios que involucraron a 1925 participantes (dadas madre-lactante). Se encontraban disponibles los datos de ms de dos estudios
clnicos para slo 8 de 64 medidas de resultado. Se observaron efectos positivos estadsticamente significativos del contacto temprano piel a piel en la lactancia
a partir de uno a cuatro meses despus del nacimiento (10 estudios clnicos, 552 participantes) (odds ratio [OR]: 1.82, intervalo de confianza [IC] del 95%:
1.08 a 3.07) y en la duracin de la lactancia (siete estudios clnicos, 324 participantes) (diferencia de medias ponderada [DMP]: 42.55, IC 95%: 1.69 a 86.79).
Se observaron tendencias de mejoras en las puntuaciones globales en cuanto al amor y el contacto afectivo materno durante la lactancia observada (cuatro
estudios clnicos, 314 participantes) (diferencia de medias estandarizada [DME]: 0.52, IC 95%: 0.07 a 0.98) y en relacin con la conducta de apego materno
(seis estudios clnicos, 396 participantes) (DME: 0.52, IC 95%: 0.31 a 0.72) con el contacto temprano piel a piel. Los neonatos del grupo de contacto piel a
piel lloraron por un perodo ms corto (un estudio clnico, 44 participantes) (DME:8.01, IC 95%: 8.98 a 7.04). Los neonatos prematuros tardos tuvieron
una mejor estabilidad cardiorrespiratoria con el contacto temprano piel a piel (un estudio clnico, 35 participantes) (DME: 2.88, IC 95%: 0.53 a 5.23). No se
encontraron efectos adversos.

El contacto piel a piel entre la madre y el recin nacido en el nacimiento reduce el llanto, mejora la interaccin del recin nacido y la madre, mantiene al
neonato ms templado y ayuda a las mujeres a obtener resultados satisfactorios en la lactancia.
En muchas culturas, es comn que se sostenga a los recin nacidos desnudos contra el pecho desnudo de la madre apenas nace. Histricamente, esto era
necesario para la supervivencia del recin nacido. En los ltimos tiempos, debido a que una mayor cantidad de recin nacidos nacen en hospitales, en algunas
sociedades se separa a los neonatos o se los viste antes de ser entregados a sus madres. Se ha sugerido que, en las sociedades industrializadas, las rutinas de
hospital pueden perturbar de manera significativa las interacciones tempranas de la madre y el recin nacido y pueden tener efectos dainos. La revisin se
llev a cabo para ver si el contacto temprano piel a piel entre la madre y el recin nacido tena alguna repercusin en la salud de neonato, el comportamiento
y la lactancia. La revisin incluy 30 estudios que involucraban a 1925 madres y sus recin nacidos. Este estudio demostr que los recin nacidos interactuaban
ms con sus madres, permanecan ms clidos y lloraban menos. Exista una mayor probabilidad de que los recin nacidos recibieran lactancia materna, o
que la recibieran durante ms tiempo, si tenan un contacto temprano piel a piel. Tambin era posible que los recin nacidos presentaran ms probabilidades
de tener una buena relacin temprana con sus madres, pero esto fue difcil de medir.
