meeting:
A letter was sent to both Lissette and her parents inviting them to the
meeting. Lissette and her parents are attending the meeting.
Age Appropriate Transition Assessments and Summary
Personal Interests:
Lissette has interest in living on her own outside of her house and becoming
more financially independent. She is a social young lady who only wants to
improve her social skills with better oral communication skills. She loves
hanging out with her sister, walking in the park, and meeting new people.
This information was obtained through informal and formal interviews with
Lissette as well as direct observation from her teachers.
Training/Education:
Lissette is very interested in attending Montgomery County Community
College, with some other students from her Work Study Skills class. She is
currently performing well below grade level in math, reading, and writing.
Her cognitive and oral language skills are below her peers and the main
focus of her academic career is now directed toward independent living
skills, which she does well in. This information was received from school
performance measures, IQ tests, and achievement tests.
Employment:
Lissette is very interested in getting a job in the food service, cleaning, or
laundry industry. She wants to be able to manage her own money and be
able to communicate with co-workers and other people. She has previously
participated in lots of unpaid training experiences through her school, as well
as worked efficiently at her job site, as noted from on job evaluation.
Independent Living Skills:
Lissette wants to live independently one day and hopes to have a family and
get married. She hopes that she can go to school on her own, which will
require her using public transportation on her own. She also wants to handle
her own finances and be able to support herself; she has previously only
managed small amounts of money, which she earned from doing chores
around her house. She currently can walk with a cane as reported by her
physical therapist, but needs mobility improvement.
Student Strengths, Preferences, Interests
Who provided information about student strengths, preferences,
interests:
The student provided us with all information about her interests and goals.
Her parents supported all of her goals and interests and built upon those
goals with things that need to meet before she can complete those things.
Her special education teachers provided us with all of her academic
information and testing scores.
Was invitation of an outside agency considered? Were they invited?
Did they attend?
Ms. Julie Potter from the program at Montgomery County Community College
was invited and attended this meeting in order to provide information and
forms for Lissettes journey for attendance there.
Mr. David Barry, the manager of taco shop, was invited and will be attending
the meeting. He is coming and will be explaining the jobs he will be having
Lissette do while she is working for him. All jobs she complete will count as
volunteer hours for her college application and will also count as credit for
her Home Economics class.
Measurable Post-Secondary Goals
Training/Education Goal:
After graduation Lissette will attend Montgomery County Community College,
where she will focus specifically on taking courses directed toward her
success in the food service.

Training/Education Connected IEP Annual Goal:


Given direct instruction and a task analysis for completing a college
application, Lissette will complete her application to Montgomery College by
March 1, 2017.

Lissette will shadow a student in the Montgomery County Community College


twice a week for the entire school year (August 1, 2017-May 1, 2017).

Employment Goal:
After graduation from Montgomery County Community College Lissette will
get a job working as a prep chef at a local/fast food restaurant.

Employment Connected IEP Annual Goal:


By May 1. 2017 Lissette will use her voice output advice 10 times accurately
to have a scripted conversation with another student in her class.

Lissette will go to Taco Shop twice a week for 1 hour and complete a list of
10 cleaning duties as directed by the manager with 100% accuracy.

Independent Living Goal:


After graduation from high school Lissette will use public transportation
independently.

Independent Living Connected IEP Annual Goal:


By January 1, 2017 given whole task instruction and a task analysis Lissette
will use directions to walk to the bus stop independently 10/10 times.

By March 1, 2017 given a bus schedule with a map Lissette will select the
correct time/stops she must take in 10 different scenarios with 9/10 accuracy.

By May 1, 2017 Lissette will use a task analysis to ride the bus independently
every day for an entire month (28 days) with 100% accuracy.
