Affidavit of Non-Employment

I, Phoebelyn Baddo Sabado, of legal age, with postal address at FD 037A, Tebteb,
Brgy. Balili, La Trinidad, Benguet, after being sworn to in accordance with the law do
hereby depose the following:

1) That as of the present time, I am not employed in any capacity at

any private or government corporation, company, and to any individual
person;

2) That I have applied for the correction of my records specifically my gender

from Male to Female in the NSO Birth Certificate;

3) That I am executing this Affidavit of Non-Employment to attest to the

truth of the foregoing and to support my application for the correction of my
records specifically my gender from Male to Female in the NSO Birth
Certificate.
