TO THE IMMEDIATE SUPERIOR/DEPARTMENT MANAGER:

Form 1 (GOAL SHEET) shall be used to document the Key Result Areas (KRA) and corresponding work
goal(s) that you intend for the staff named above to do and accomplish within the agreed covering period.

How to accomplish this form:

1. In discussion with the staff, refer to the current Job Description or Work Instructions; identify and agree
on the KRA(s). Mandatory KRA(s) are Competence, Client Focus and Concern for the Team.
Other/additional KRA(s) may be determined and added.
2. Under each KRA, formulate the mutually agreed S.M.A.R.T. Work Goal(s).
3. Align each work goal to the department/project objectives and operational directions.
4. Determine the Factor Weight for each KRA; Total of all the Factor Weights = 100%.
5. Schedule a review meeting with the staff. The review meeting is done at the end of every job or project
assignment (for short term), or every 6 months (for long term job or project assignment).

Goal(s):
Finish 7 sites /week regardless of the structure complete with structural analysis, computation,
retrofitting recommendation and report.

Responsible in the finalization of drawings for sites recommended for retrofitting thru instruction,
monitoring and checking. Preparation of retrofitting report.

Site inspection and preparation of sketches and draft inspection report

PERFORMANCE STANDARDS. This will be rated during the Review Meeting. Summary of actual accomplishments and
ratings will be reflected in the Performance Output Summary Sheet.

Maintains accuracy and timely submission of outputs.

Helps in the final preparation of the structural report to meet timeline with client.

Liaises with Globe field officers and cell site building owners /administrators
