I, CONCHITA CO, Filipino, of legal age, single, a resident of 874

Tres De Abril St., Cebu City after having been duly sworn in accordance
with law, do hereby depose and estate:

1. That I am the Treasurer of Broadway One Shipping

Corporation: (Broadway for brevity), a corporation duly
organized and existing under the laws of the Philippines
with principal office address in Cebu City;

2. That one (1) booklet of Bill of Lading of Broadway with

series Nos. 00051-00100 was misplaced and I cannot
anymore remember where I put the same;

3. That this matter came known to me last June 28, 2014

when I was about to do my routine reports;

4. That I tried to exert earnest efforts and due diligence to

locate the same, but all to no avail, and now considered
beyond recovery;

5. That this affidavit is executed to attest the truth and

veracity of the foregoing facts stated herein and for
whatever purpose and intent it may serve best;
