AFFIDAVIT OF ADMISSION OF PATERNITY

I, RANDY Y. DAVID, of legal age, married, Filipino and a resident of Brgy.

Cut-Cut 1st, Capas, Tarlac, after being duly sworn in accordance with the law,
hereby deposes and states THAT:

1. Sometime in the year 2000, I met Villa Blanca Esteban, of legal age, and a
resident of Cut-Cut 1st, Capas, Tarlac;
2. As a result of said relationship, she conceived and gave birth to a child on
July 30, 2002 an we named the child Bianca Lyka Marie;
3. I am the biological father of the child, Bianca Lyka Marie;
4. At the time of birth of said child, I am not yet legally married to Villa Blanca
Esteban, her biological mother;
5. I hereby admit under oath my paternity of the child, Bianca Lyka Marie;
6. I am executing this affidavit to attest to the truth of the foregoing facts and
for whatever legal purposes it may serve.
