PRENATAL CHECK-UP

General Objectives:

Within 4 hours of exposure at Day-as Barangay Health Center, student nurse will be
able to apply the theoretical concepts learned in Health Assessment, Community
Health Nursing and Maternal, especially in assessing the mother, building rapport
with the mother, performing Leopolds maneuver and auscultating fetal heart rate.

Specific Objectives:

Within 4 hours of Health Center Exposure, the student nurse will be able to:

Establish rapport among clients and the rest of the healthcare staff by
introducing oneself.
Take the in-depth history such as activities and practices of the client for the
past month that may affect the development of her pregnancy
Assess for high-risk pregnancy conditions such as clients with Pregnancy
Induced Hypertension or Gestational Diabetes and give health teachings to
avoid further complication of the condition.
Perform efficiently the skills of Leopolds Maneuver, auscultating the fetal
heart tone and assessing the fundal height through the use of McDonalds
Rule
Utilize properly the materials and facilities that the barangay health center
has to offer for a better assessment and check-up of every client
Individualize the care to each pregnant client and cater to all the clients
needs that are within the student nurses competency level
Deliver the benefits of having a healthy diet, regular exercise and correct
intake of vitamins or supplements during pregnancy.
Explain the importance of certain vaccines such as the Tetanus Toxoid Vaccine
both for the health of the mother and her baby.
Gain knowledge and confidence from her experience so that she can be a
more competent student nurse who can exercise critical thinking in a short
span of time.

Evaluation:

After 4 hours of being exposed in Barangay Day-as Health Center, I was oriented to
the barangay health center and its healthcare staff which is composed primarily of a
midwife and her assistant. I was also oriented on how to go about the prenatal form
which comprises their basic information, the vaccines they received, their diet and
lifestyle for the past months, their urinary and bowel elimination, the medication
they have taken in and unusual conditions that they might have encountered during
the past few days and how they managed it.

Recommendations:

My recommendations would include a proper orientation to the student nurses


about the activities offered by the barangay health center and how to go about each
services. Also, the waiting area should be cleaned daily to prevent the accumulation
of dust inside the room. Extra chairs and electric fans would also be helpful for a
better room ventilation to promote comfort among the clients.

Evaluation:

After 4 hours of being exposed in Barangay Day-as Health Center, I was able to gain
more confidence in performing the Leopolds maneuver and also overcame my fear
of pushing the pregnant uterus of the clients too hard. I also managed to locate and
auscultate the fetal heart tone with the use of an ordinary stethoscope in contrary
to what we usually see at the hospital which uses a Doppler fetal monitor.

Recommendations:

My recommendations include a broader explanation to each client about what is


expected and what should be done on each stage of the pregnancy. Also, to
promote long term learning, the health center may provide them with handouts or
pamphlets that they can read from time to time at their respective homes. Also, the
staff can rearrange the office so that the examination room can be bigger. They can
remove unnecessary things inside like lessen the number of tables and also the
cooking stove inside. Lastly, I suggest that the midwife should be able to build
rapport among clients since I have observed that some clients are closed off and
cannot express properly their thoughts whenever they are asked certain questions
by the midwife.
