RANKING OF QUALIFIED ELEMENTARY AND SECONDARY TEACHER I

APPLICANTS for SCHOOL YEAR 2016-2017

School Heads / TIC (Elementary and Secondary Schools)

1. To facilitate the hiring of new teachers for School Year 2016-2017 the field is hereby informed that
DepEd Order No. 22, s. 2015: Hiring Guidelines for the Remaining Teacher Positions shall be
apply to the filling up of newly created/natural vacancies for Teacher I positions in the public
elementary and secondary schools.

2. The field is hereby directed to submit the School Registry of Qualified Applicants and Individual
Evaluation Sheet together with the supporting documents to the Division Selection Committee on
January 30, 2016.

3. The City Schools Division of Dasmarifias Selection Committee may refuse acceptance of the
application if any of the following document is not submitted.
a. CSC Form 212 (Revised 2005) in two copies with latest 2"x2" ID picture, Unique
Applicant Number (UAN) must be indicated at the upper right corner of the Personal
Data Sheet. New applicants shall register to Department of Education's online system
at application.deped.gov.ph .
b. Certified photocopy of unexpired PRC professional identification card (Claim stub
will not be accepted) or a PRC Certification showing the teacher's name, LET
Rating, and other information recorded in the PRC Office
c. Certified photocopy of ratings obtained in the LET/PBET
d. PRC online verification of Professional Licenses by license no.
e. Certified copy of transcript of records (For employment purposes)
f. Computation of General Weighted Average
g. Copies of service records performance rating/s and school clearance for those with
teaching experience. If unavailable, the applicant must submit a justification citing
the reason/s for unavailability.
h. Certificates of specialized training, if any
i. Certified copy of the Voter's ID / Voter's Certification and/or any proof of residency
j. NBI Clearance (not expired)
k. Omnibus certification of authenticity and veracity of all documents submitted, signed
by the applicant.

4. Applicants in Registry of Qualified Applicants School Year 2015-2016 who are not
appointed to a permanent position will be considered new applicants.

5. Teacher applicants who have not practiced their profession for th past five (5) years shall
be required to take at least twelve (12) units of education courses consisting of at least six
(6) units of pedagogy and six (6) units of context courses before ihey will be included in the
registry of qualified applicants.

6. Schedule of the Division demo teaching, interview and English Proficiency Test to all
qualified applicants will be announced after the review and 4onsolidation of Individual
Evaluation Sheet.
