he DFA enforces strict rules for personal appearance. All passport applicants with appointment
chedules, aged 0 years onwards, must appear physically at the DFA on the specified date and time
or biometric and image capturing. Absolutely no chaperone will be allowed for passport applicant:
8 years old and above.
1. Your call will be received by a Customer Care Officer who will assist you in setting an appointment fo
your passport application.
2. After getting your appointment schedule, you will be asked to provide your personal information for
the passport application.
3. You will be advised on the applicable requirements you need te bring for your passport application,
You will receive a reference number relative to your appointment schedule.
5, Go to passport com phiprint to print your application form using the Print Code given by the Custome
Care Officer.
6. New passport applic ants will be asked for a copy of their NSO Birth Certificate. Please make sure
that you have one before your appointment date.
7. On your appointment date, proceed to the DFA Consular Office located at ASEANA Business Park,
Bradco Avenue comer Macapagal Boulevard, Paranaque City (see locator map). You must be at the
DFA Consular Office 30 minutes before your scheduled time. Early/Late comers will not be
entertained.
Present your printed application form to the Appointment Counter.
9. Get your queue number at the Information Counter and sit on the waiting section of the Processing
‘Area. Wait for your queue number to appear on the monitor indicating what processing window you wi
goto.
10. After processing, proceed to the Cashier's area located at the 2nd Floor. Pay the applicable passport
processing fees (see passport fees)
11. After payment, proceed to the Encoding Section for data capturing.
12. After your photo is captured, personal details and biometrics are encoded, you have the option of
a) claiming your passport at the DFA consular office where it was applied at; or
b) have it delivered by courier to your home.
If availing of the delivery, proceed to the Delivery Counter to pay for the delivery fee.

4. Check your email for your reference number and a link to print your application form. You will need to
bring the printed application form to the DFA on your appointment.
