NUR 3205: Advancing Nursing Practice

Self-Care Assignment
Purpose: The purpose of the self-care assignment is to appreciate ones self-care as foundational
to caring for others.

Objectives:

1. Reflect on self-care as a component of caring for others.


2. Develop and implement a self-care plan.
3. Start/enhance ones own self-care.
4. Appreciate mental readiness as part of behavior change.

Evaluation: The assignment will be completed throughout the semester. The student will
complete the three components and follow the instructions below to submit each assignment.
The assignment will be evaluated by the following rubric.

Week 2: 1) Complete Testwell's Holistic Lifestyle Questionnaire
Create Blog College Assessment
and Begin! (http://www.testwell.org/twfree.htm) then answer 10
the following questions for your first entry in your
Self-Care Blog (link located in Bb under Module I):
a. Based on your test results, what behavior
do you wish to change?
b. Identify 3 personal benefits to changing this
behavior.
c. What is your long term goal for this
behavior change? (ex: run a 5k by January
2015)
d. What are your short term goals that will
help you achieve your long term goal
(identify at least 3)? (ex: develop exercise
plan, make exercise a daily priority, etc)
e. Reflect on the Behavioral Change Theory
and identify what stage you are in and
briefly state why you chose this particular
stage. Ex: pre-contemplation,
contemplation, etc.)
f. What barriers if any do you think you will
have in reaching your goal?
2) Read blog of one other peer and comment. Suggest
at least 2 possible strategies that may help them be
successful in attaining their goal.
Weeks 3-14 1) Make a blog entry regarding your progress at least 8 10 points per entry = 80 points
of the remaining weeks in the course. Your entry
each of these weeks should address the following:
a. What I planned to do to contribute to the
attainment of my goal this week.
b. What I actually did..
c. What helped me follow my plan
d. What obstacles did I face this week
e. What should I do differently next week
2) Read blog of one other peer and comment. Suggest
at least 1 possible strategy/suggestion that may
help them in the upcoming week.

Week 15 FINAL BLOG: 10 points
1) Tell us how you did! Reflect on your progress toward
your goal and what you can do differently in the
future. If successful goal attainment, what do you
think was the reason you were successful?
2) Read blog of one other peer and
comment/congratulate/encourage!

TOTAL= 100 POINTS
