You are being issued this first and last written warning for failure to maintain good quality
lessons based on the following incidents, to wit:

a) Over 1% of without-notice-cancellation rate in the term from


June.6 to June.20, 2014

This is clearly written in the Official Announcement No. 259 Very Important:
Suspension Penalty on Cancellation Without Notice connected with your Contract for
Services and the Teachers Practical Guidelines. Commission of a new or related violation of
the contract and the guidelines, and/or continued unsatisfactory performance will lead to the
suspension of your account from the teachers list.

I acknowledge by my signature below that I have RECEIVED, READ, and UNDERSTOOD the contents stated in the
First and Last Written Warning.
