Indicate the method used to invite the student to the transition meeting: His
special education teacher invited him in person to attend the transition
meeting. That way Jason feels comfortable knowing what he's about to do,
also so he can inform his parents and teachers what he would like to do for
his future.

Age Appropriate Transition Assessments and Summary


Personal Interests:Jason loves to to watch sports basketball, hockey, or
football on TV or in person. With the money he earned from his summer job
last year, he bought season tickets to see the local NBA team. He is also the
treasurer of the student council and plays racquetball in a community league
with some of his friends. Along with that he is an active member of the youth
group at his church.

Training/Education: Jason should do some observations of teachers to


understand how they perform their task as a teacher before, during and after
class. Along with observations Jason should interview some of the teachers
to get an understanding of why they became a teacher and what advice they
can give him on his path ahead of him. Also so he knows what college he
wants to go to he should visit at least one college with a teacher preparation
Program.

Employment: On Saturdays during the school year, Jason earns spending


money by working in his uncles legal firm by answering the telephone and
filing various documents.

Who provided information about student strengths, preferences, interests:
Jason himself provided the information about his preferences and interests.
While his parents and teachers added to both as well as give an insight in his
strengths inside and outside of school.

Was invitation of an outside agency considered? Were they invited? Did they
attend?
Yes an invitation was considered for the disability services office of two state
colleges and vocational rehabilitation, it was sent but unfortunately due to
prior commitments they could not attend.

Measurable Post-Secondary Goals


Training/Education Goal: After graduating from high school, Jason will attend
the University of Arizona to obtain his undergraduate degree in math and
education, to become a high school math teacher.
Training/Education Connected IEP Annual Goal: Jose will be given a
short story, Jason will identify the components of a story (such as the
setting, characters, problem, solution and resolution to the problem)
with 85% accuracy.

Employment Goal: After completing high school Jason will discuss with his
uncle to increase his hours from just Saturday's to become a part time
worker. Another idea if that doesn't work is to become a paraprofessional at
a high school to both gain experience as a teacher and gain employment,
which can transfer to him working there after he graduates from college.

Employment Connected IEP Annual Goal: Jason should participate (if


the distinct has one) a teacher mentor program. Jason will also write an
essay about what he expects to happen for his future career as an
educator such as his goals, what he views as negatives and positives
of teaching, his motivations and his drive to become a teacher. In his
essay about his mentor program experience he will have a score of
85% or better accuracy in both grammar and spelling by December
2016.

Independent Living Goal (When necessary): Jason plans to live in the dorms
with many other first year students. During his last IEP meeting, his parents
discussed the services on college campuses available to students with
disabilities.
