AFFIDAVIT OF ABANDONMENT

I, JULIANNE RITZ B. SALAZAR of legal age, married, Filipino, and a

resident of San Isidro Norte, Binmaley, Pangasinan, after having been duly
sworn to in accordance with law, hereby depose and say that:

1. I am the biological mother of twins SEAN DEVIN S. PETERS and MISHA

FIONA S. PETERS.

2. The biological father of my children is George Henry A. Peters.

3. My children were born on April 29, 2006, at Saint Louis University

Hospital in Baguio City.

4. George and I were college sweethearts. However, our relationship

turned sour when the pressure of young parents set in.

5. We tried to fix our relationship but on 2007, he finally abandoned us.

6. After the abandonment and up to the present I, the undersigned has

been in sole custody and care of the above mentioned two minors
giving them moral and financial support for their education and well
being.

7. I attest to the truth and veracity of the foregoing statements for all
legal intents and purposes it may serve.
