AFFIDAVIT OF ACKNOWLEDGMENT OF PATERNITY

I, ALFREDO MICHAEL B. MATALOTE II, of legal age, Filipino,

married, and presently residing at Cararayan, Naga City after having been
duly sworn to in accordance with law, do hereby depose and say:

1. That I am the biological father of the child MICHAELLA MAE PERICO;

2. That the biological mother of said child is REBY B. PERICO;

3. That our child was born on June 3, 2005 at Binagasbasan,

Garchitorena, Camarines Sur;

4. That at the time of the birth of our child, I was not legally married to
her biological mother;

5. That I hereby acknowledge my paternity of the child, MICHAELLA

MAE PERICO; and

6. That I execute this affidavit to attest to the truth of the above facts and
for whatever legal purpose it may serve.
