AFFIDAVIT OF DISCREPANCY
I, ANTONIA ALUMIT SIMSIM, of legal age, widow, Filipino Citizen and
with residence and postal address at Naguey, Atok, Benguet, after having been
duly sworn to in accordance with law hereby depose and state that:

1 I am the wife of the late KIGUIL LUCAS SIMSIM, a war veteran and
member of the PVAO;

2 He died last October 25, 2016 at Naguey, Atok, Benguet as evidenced

by his Certificate of Death;

3 However, a lot of discrepancies appears as to his records and as to my

name, to wit:

a. In his PVAO FORM and DEATH CERTIFICATE (DC) his name is

KIGUIL LUCAS SIMSIM, however, in his MILITARY SERVICE
RECORD (MSR) his name is KIGUIL S. SIMSIM;

b. The name of his father in his PVAO FORM and MARRIAGE

CONTRACT (MC) is COYAM-AN SIMSIM, however, in his Death
Certificate his fathers name is spelled as KUYAM-AN SIMSIM;

c. The name of his mother in the PVAO FORM is SELING LUCAS,

in the Death Certificate is CELEN LUCAS and in the
MARRIAGE CONTRACT is CELING LUCAS;

4 The correct name of my husband is KIGUIL LUCAS SIMSIM as

recorded in his PVAO FORM and DEATH CERTIFICATE. The reason
why in his MILITARY SERVICE RECORD his name is KIGUIL S.
SIMSIM is that the one who entered his name in the said record
inadvertently wrote S as middle name copying that of the family
name SIMSIM. Thus, the name KIGUIL LUCAS SIMSIM, as per
PVAO Form and Death Certificate and KIGUIL S. SIMSIM as per
Military Service Record, pertains to my late husband as one and
the same person;

5 As to the name of his father, the correct spelling is COYAM-AN

SIMSIM as recorded in the PVAO Forms and Marriage Contract. In
the Death Certificate the spelling is KUYAM-AN SIMSIM which is
wrong as the typist who entered the name inadvertently spelled as
such maybe due to wrong information or details given by the member
of the mourning family. Hence, the name COYAM-AN SIMSIM as per

 PVAO Form and Marriage Contract and KUYAM-AN SIMSIM as per
Death Certificate, pertains to one and the same person;

6 As to the name of my husbands mother, the correct spelling is

CELING LUCAS as written in our MARRIAGE CONTRACT. In the
PVAO FORM the spelling is SELING LUCAS, maybe at that time
they just dictate the name and the one typing inadvertently just
spelled as such without clarifying as to the correct spelling. Likewise,
in the Death Certificate of my husband his mothers name is spelled
as CELEN LUCAS, the reason is that during that time our family is
still mourning and still composing our thoughts, thus, when they
asked for the information or details regarding my mother-in-laws
name, we just dictate the name without clarifying the correct spelling.
Hence, the name SELING LUCAS as per PVAO FORM, CELEN LUCAS
as per Death Certificate and CELING LUCAS as per Marriage
Contract, pertains to one and the same person;

7 With regard to my name ANTONIA ALUMIT SIMSIM, my middle name

ALUMIT is my fathers first name, ALUMIT BAYEC. We Igorots are
known for having the privilege of choosing what we will use as our
family name as part of our traditions and customs. My parents
preferred to use ALUMIT as my family name to preserve the legacy
of such name and so that it will not be forgotten. Thats explains why
my family name is ALUMIT instead of BAYEC.

8 The aforementioned discrepancies are explainable and attributable to

sheer oversight and/or honest mistake and are not tainted with any
deliberate malicious or criminal intent, but due only to
aforementioned circumstances;

9 I am executing this affidavit to attest to the truth of my foregoing

statements and to support the transfer of pension of my late husband
in my name and for all legal intents and purposes it may serve.

age, Filipino Citizens and residents of Naguey, Atok, Benguet, hereby CERTIFY
under oath that the affiant signed this instrument through her RIGHT THUMB
