AFFIDAVIT OF DISCREPANCY
I, LARRY B. CASUPANAN, of legal age, single, Filipino and a
resident of Bulawan, San Miguel, Zamboanga del Sur, Philippines, having
been duly sworn in accordance with law, hereby depose and state:

1. That as shown in the Certificate of Live Birth of my father recorded in

the Office of the Civil Registrar of Pampanga, it is stated that his name
is PAMPILO MANALANG CASUPANAN;

2. That in my Certificate of Live Birth as recorded in the Office of the Civil

Registrar of the Municipality of San Miguel, it was inadvertently and
erroneously written that the name of my father is PANFILO MANALANG
CASUPANAN;

3. That the names PAMPILO MANALANG CASUPANAN and PANFILO

MANALANG CASUPANAN refer to one and the same person and that his true
and correct name is PAMPILO MANALANG CASUPANAN;

4. That I am executing this Affidavit of Discrepancy to attest

to the truth of the foregoing facts and to explain the discrepancy in my
Certificate of Live Birth and of my fathers Certificate of Live Birth as
regards to my his name;
