age, Filipinos, and residents of Davao City, after having been duly sworn to
in accordance with law, hereby depose and say, THAT:

1. We personally know JOHN PAUL Q. MAKABUGTO, born on July

15, 1990 a resident of Davao City and is employed at the Department
of Environment and Natural Resources (DENR);

2. We have known JOHN PAUL Q. MAKABUGTO being our long-

time family friend and neighbor;

3. We are stating as a fact that to the best of our knowledge, that the
DENR employment records of JOHN PAUL Q. MAKABUGTO
erroneously states that his name is JHONN PAUL Q.
MAKABUGTO;

4. We are stating as a fact to the best of our knowledge that JOHN

PAUL Q. MAKABUGTO and JHONN PAUL Q.
MAKABUGTO are one and the same person

5. We are executing this instrument to attest the truthfulness and

correctness of the above stated facts to the best of our personal
knowledge;

AFFIDAVIT OF LOSS

I, JEJOMAR Z. ROXAS, of legal age, single, a Filipino citizen and

a resident of 422, Leon Garcia, Agdao, Davao City after having been sworn
to in accordance with law, do hereby depose and say that:

I am employed at the Bureau of Fire Protection of Davao City;

That sometime in May 2017, my wallet along with my Drivers

License, issued in my name with License No. L99-24-32243224 was
missing;

That after we had assiduously conducted a thorough and exhaustive

search to my Drivers License, but to no avail. I finally gave up the
said license as forever lost and beyond recovery;

For all legal intent and purposes, I am convinced that my Drivers

License, issued in my name with License No. L99-24-32243224 has
been lost forever and is now beyond recovery under the
circumstances;

I am executing this affidavit to attest to the truth and veracity of the

foregoing facts, to swear that the foregoing assertions are the true
account of the events as they happened and there is no fraud or
misrepresentation involved in the loss thereof, to inform my
employers of the true circumstances of the loss of my Drivers
License, and to faithfully attest that this former was not used or issued
by me for any payment or other consideration by any person or entity.

I, JEJOMAR Z. ROXAS, of legal age, single, a Filipino citizen and

a resident of 422, Leon Garcia, Agdao, Davao City after having been sworn
to in accordance with law, do hereby depose and say that:

1. I am the father of JUNJUN Z. ROXAS, 15 years old, who is charged

with Viol. of PD 1619 before the City of Manila, Regional Trial Court
(Br. 43);

2. The above named child is under my custody and care at the time of his
arrest allegedly for the said offense;

3. I am ready and willing to assume and fulfill my duty and responsibility to

supervise and guide the said child during his release on recognizance
pursuant to the provisions of the Child and Youth Welfare Code and
the Family Courts Act of 1997;

4. I am likewise solemnly promising and undertaking before this Honorable

Court to produce and present the said child before this Honorable Court
whenever required to do so during the pendency of the above-cited case
in order to answer for the said charge.
