(Re: Drivers License Card)

I, DOMINIC EMBODO, of legal age, Filipino, single, and a resident

of Poblacion, Matanao, Davao del Sur, Philippines, after having been sworn
to in accordance with law, do hereby depose and state that:

1. I am a holder of a drivers license identification card issued by

Land Transportation Office (LTO);

2. I used to keep my identification card inside my cabinet at my

residence. On 14 June 2017, I realized I had misplaced my said LTO
identification card;

3. I searched for it in the cabinet in every nook of the house with the
aid of the members of my family. To our dismay, we could not find my LTO
identification card;

4. I am executing this affidavit to apprise the concern government

agencies especially the Land Transportation Office (LTO) and for whatever
legal purpose this may serve best.

AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, VANESSA REGIDOR, of legal age, Filipino, married, and a

resident of Buas, Matanao, Davao del Sur, and JOLLY MAE NAVASCA,
of legal age, Filipino, married, and a resident of Bansalan, Davao del Sur,
Philippines, after having been sworn to in accordance with law, do hereby
depose and state that:

1. We personally know the person of DOMINIC ELOJA EMBODO

who was born on 07 March 1989 in Matanao, Davao del Sur, Philippines;

2. His middle name erroneously appeared in his Social Security

System (SSS) identification card as ALOJA instead of ELOJA, the true and
correct one pursuant to his Certificate of Live Birth;

4. The middle names ALOJA and ELOJA refer to one and the same
person;

5. We know of this fact and circumstance because we have been long

time family friends and associates; and

6. We execute this sworn statement to attest to the truth of the

foregoing and for whatever legal purpose this may serve best.

AFFIDAVIT OF UNDERTAKING

I, DOMINIC EMBODO, of legal age, Filipino, single, and a resident

of Matanao, Davao del Sur, Philippines, after having been sworn to in
accordance with law, do hereby depose and state that:

1. I am the vendee of a parcel of land covered by Tax Declaration of

Real Property No. 11-0007-00290 located at Ceboza, Matanao, Davao del
Sur, consisting an area of Twenty Thousand (20,000) square meters, more or
less pursuant to a Contract to Sell of a Parcel of Land dated 05 January
2017, appearing in the notarial registry of Atty. Vanessa E. Regidor, a notary
public of Matanao, Davao del Sur, Philippines, as Doc. No. 305, Page No.
61, Book No. 21, Series of 2017;

2. I undertake to fully settle the remaining balance to JOLLY MAE

N. NAVASCA amounting to One Million Two Hundred Thousand
(PHP1,200,000) on or before 30 August 2017;

3. With the conformity of the vendors appearing below, the deadline

stated in the Contract to Sell for us to settle in full the consideration of the
conveyance is deemed amended and extended;

4. I am executing this affidavit in order to attest to the truth of the

foregoing and for whatever legal purpose this may serve best.
