AFFIDAVIT OF DISCREPANCY

I, FLORAMY RAMIREZ PARREO, of legal age, married, Filipino citizen

and a resident of zone 1, Boalan, Zamboanga City, Philippines, after having
been duly sworn to in accordance with law, hereby depose and say that:

1. I am a holder of a Certificate of Registration and Official Receipt

bearing numbers 237823983 and 1084690121, respectively, duly
issued by the Office of Land Transportation, Zamboanga City;

2. However, due to advertence and/or oversight there was a discrepancy

as appearing in the record of the Certificate of Registration and
Official Receipt, particularly on my surname FLORAMY RAMIREZ
PARRENO;

3. I make these statements under oath and in good faith in order to

establish the fact that my true and correct surname is FLORAMY
RAMIREZ PARREO as appeared on my marriage contract issued by
the Office of the Civil Registrar at Zamboanga City and that
FLORAMY RAMIREZ PARRENO are one and the same person;

4. I hereby executed this affidavit to cause the necessary correction of

my true and correct middle name remains, up and until present as
FLORAMY RAMIREZ PARREO and that FLORAMY RAMIREZ
PARRENO are one and the same person;

5. I execute this Affidavit to attest to the truth of the foregoing facts

based on personal knowledge and authentic records and for whatever
legal purpose it may serve.
