AFFIDAVIT OF DENIAL

I, Charlie Magne J. Crabajales, of legal age, Filipino, single and resident of #062 Akle street, Nova
Tierra Lanang, Davao City, after having been duly sworn to in accordance with law, depose and state:

1. That I was born on February 11, 1984 in Davao City, Philippines;

2. That to this year 2015, I have received a writ of subpoena from the agency of B.I.R. requiring

me to verify some facts contain the writ.

3. That in the writ contains a series of questions regarding my tax liabilities from the year 2008

to 2015.

4. That after I have received the writ and answer the facts and issues involved, I have send

the person with my consent and authority to settle the issue that requires my answer.

5. That the B.I.R. insists that since 2008 to 2015, I still have a pending unpaid tax from various

transactions regarding purchased items from abroad and the series of annual tax due in

employment.

6.
