Herald patch : first sign that appear circleshape in chest, stomache and getting bigger
until back.
Makula Eritem : Lesi yang berbatas tegas berwarna kemerahan karena pelebaran pembuluh
darah.

1. Why there are herald patch and makula eritem ?

- Herald patch : one of the viral infection, and the first lesi that appear from the disease.

2. Whats the diagnose from this scenario? DD? Symptom of the diagnose?

Diagnose =
- pitiriasis rosea= ( eritopapuloskuamosa with herald patch like pineapple tree )
terapi : topikalbedak asam salisilat untuk mencegah infx dan penumpukan keratinosit.
simtomatik CTM sifatnya sedatif ( menghindari garukan karena sifatnya sedatif )
Antihistamin diberikan bila ada rasa gatal
Kortikosteroid menghilangkan rasa gatal dan lesi ( bila penyakit < 1 bulan)
- d. Seboreik = kaya akan kelenjar sebasea
- Psoriasis= ( stratum korneum berinti bersisik )
- Eritroderma= (from the makula eritem)
- Dermatofitosis =penyakit karena jamur, lesi pada herald patch tepinya lebih meninggi.
Symptom : lesi merah, batas tegas, central healing ( tepi lesi gatal tengahnya bagian
penyembuhan karena jamur dermatofitosis yang disebut tinea.Jamur memakan keratin
dari tengah ke tepi)
Predileksi : di daerah lipatan
Tinea corporis = dermatofitosis pada daerah lengan,tungkai,badan karena 3
genus jamur ( epidermophyton, trycophyton,microsporum)
Terapi
Topikal salep whitfield ( as. Salisilat,as.benzoat,imidazol,
ketokenazol,piroksalamin siklik)
Sistemik- Antihistamin ( griseofulvin, itrakonazol,ketokonazol)
Pitiriasis versicolor(PV) / Panu non dermatofit
Terapi
Ketokonazol dan itrakenazol

- Sifilis = cirinya sama seperti skenario tanpa herald patch

3. Causes of scale skin?

 Terjadi parakeratosis ( penebalan keratin dan masih berinti ---bersisik)
Kepanasan , terkena air lama, pakaian terlalu ketat---Dermatofitosis.
Penyebab passti belum ada, paling sering karena pergantian cuaca.Didahului ISPA

Diagnosis from the scenario:
Pitiriasis Rosea : penyakit kulit yang belum diketahui penyebabnya, dimulai dengan
sebuah lesi inisial berbentuk eritema dan skuama halus. Kemudian disusul oleh lesi-
lesi ynag lebih kecil di badan, lengan dan paha atas yang tersusun sesuai dengan
lipatan kulit dan biasanya menyembuh dalam waktu 3-8 minggu.

Pityriasis Rosea: skin disease of unknown cause, began with an initial lesion
erythema and skuama finely shaped. Then followed by ynag lesions smaller body,
arms and upper thighs are arranged according to the folds of skin and usually heal
within 3-8 weeks.

Symptoms
The signs of pityriasis rosea include:
A large single spot (herald patch) forms.
The herald patch is scaled and coloured red to tan.

A fine rash of small spots starts to appear within a week of the first patch appearin
g.
The spots become small oval patches that are coloured red to tan.
The patches seem crinkly and loose in the centre.
The rash may be itchy.
The upper body and upper arms are usually affected.
The patches may follow the ribs in lines.
 The rash may spread to the upper thighs.
Occasionally, the rash may spread to include the neck and lower face.
gejala
Tanda-tanda pityriasis rosea meliputi:
Adanya bercak tunggal besar (pemberita patch) bentuk.
Para pemberita patch skala dan berwarna merah untuk tan.
Ruam denda bintik-bintik kecil mulai muncul dalam waktu seminggu dari patch
pertama muncul.
Bintik-bintik menjadi patch kecil berbentuk oval yang berwarna merah untuk tan.
Patch tampak berkerut dan longgar di tengah.
Ruam mungkin gatal.
Tubuh bagian atas dan lengan atas biasanya terpengaruh.
Patch dapat mengikuti rusuk dalam baris.
Ruam bisa menyebar ke paha atas.
Kadang-kadang, ruam dapat menyebar ke termasuk leher dan wajah bagian
bawah.

4. How to prevent the disease from the scenario?
It is not possible to make the rash disappear any faster, so treatment aims to mana
ge the
symptoms. Options may include:
Avoid using soap as this can irritate the rash.
Bathe using plain water or some kind of moisturiser, such as bath oil.
Steroid creams can be used to help reduce itching.
Oral antihistamines may help in cases of severe itching.
Mild moisturising creams can be applied generously and often.

Pilihan pengobatan
Hal ini tidak mungkin untuk membuat ruam menghilang lebih cepat, sehingga
pengobatan bertujuan untuk mengelola
gejala. Pilihan mungkin termasuk:
Hindari menggunakan sabun karena hal ini dapat mengiritasi ruam.
Mandi menggunakan air biasa atau beberapa jenis pelembab, seperti minyak
mandi.
krim steroid dapat digunakan untuk membantu mengurangi gatal.
antihistamin oral dapat membantu dalam kasus gatal parah.
krim pelembab ringan dapat diterapkan murah hati dan sering.
