AFFIDAVIT DISCREPANCY

I, NASROLLAH ANDONG MANGULAMAS , Filipino, of legal age, with

postal address at Poblacion, Dalican, Datu Odin Sinsuat, Maguindanao after having
been sworn to in accordance with law, hereby depose and state:

1. That the spelling of my first name as it appears in my school credentials and

records at Notre Dame University is NASRULLAH ;

2. That the true and correct spelling of my first name is NASROLLAH as

reflected in my Certificate of Live Birth and hereto attached as Annex A;

3. That I am executing this affidavit to attest to the truthfulness and veracity of

the foregoing and for the purposes of requesting the change of information in my
school records in NDU from NASRULLAH to NASROLLAH and for whatever legal
purpose it may serve best.
