BUSINESS EXPERIENCE

Work Summary: I am currently working as the house manager in a mixed gender Max Care CRR in Chester County, PA.
I moved to that position from serving as a Behavioral Health Specialist in homeless services in Delaware County, PA.
Prior to that, I worked in targeted case management for two years. Before entering the arena of case management, I
have had eight years of experience working in the Mental Health field from the Administrative perspective. During my
schooling, I decided to seek employment in the administrative side to further my knowledge of the field. In 2002, I left
employment to concentrate on finishing my degree.

Horizon House, Inc., Chester Springs, PA


CRR House Manager 04/07 to Present
Oversee a staff of seven with five live-in residents with various degrees of mental disorders
Supervised the closing of another CRR in Chester County.
General managerial duties include:
o Maintaining OSHA requirements
o Maintaining state and county regulations for the running of the household
o Provide monthly supervision to staff members
o Prepare and submit monthly statistical reports which include:
Fiscal
Residential data for the county
Monthly program reports
Monthly heat and safety reports
o Coordinate biannual treatment team meetings and 60 day recovery plan reviews.
o Create and maintain participant recovery plans and working with the participants to achieve their goals
o Prepare monthly chart audits
o Maintain DAP notes
o Complete participant psycho-social report twice a year for each resident
o Develop and implement Behavioral Plans for the residents
When necessary, steps in to perform resident care duties such as:
o Medication counts and monitoring
o Transportation of participants to appointments
o Provide counseling, as needed

Behavioral Health Specialist 06/05 to 04/07
Carry a caseload of 15 to 20 homeless participants who have Mental Health / Drug and Alcohol issues
Assess participants for Mental Health / Drug and Alcohol needs
Aided participants in accessing local food banks and clothes closets
Connect participants to appropriate services
Assisting with the navigation of Dept of Public Welfare System and / or Social Security System

Update Magellan Behavioral Health monthly on participant progress and obtain necessary authorizations
