* Notes for Items 2 to 4 are as follows:

Online Application Type I forms applicants: Not compulsory for applicants. Applicants using Type I form only need to
submit these documents if they have indicated that they need amendments at Section 7 of the Type I form.
Online Application Type II forms applicants: Compulsory.

The documents that you are required to submit will be listed at the acknowledge page; the items that you
should mark in this checklist should also tally with the items as shown in your acknowledge page. DO NOT
submit documents that are not listed at the acknowledge page. Submitting unnecessary documents may
slow down the processing time of your application.
