The nature of the work performed at JT3 involves our Country's national security and defense.
Accordingly, please carefully review the following conditions regarding your prospective
employment with the Company.

Security Terms and Conditions of Employment

If clearance and access are required based upon your work location, you will be expected to
qualify for and maintain these requirements and standards as long as you are employed by the
Company.

Employment with the Company is not for a specific term and can be terminated by yourself
or the Company for any reason, with or without cause. This condition represents the sole and
entire Agreement that you have with the Company governing the conditions under which your
employment with the Company may be terminated. Further, your agreement to this at-will
provision is separate and distinct from any other agreement, express or implied, which you
have or may enter into with the Company regarding the terms and conditions of your ongoing
employment. No other agreement will, in any way, limit the Company's right to terminate your
employment, with or without cause. This Agreement will remain in full force and effect
notwithstanding any changes in your position, compensation, or other terms or conditions in
your employment for the full period of your employment with the Company. Any changes,
modifications, superseding, or recision of your term of employment must be in writing and
signed by yourself and an authorized representative of our Human Resources Department.
Additionally, we request that all our employees, to the extent possible, give us notice if they
intend to resign.

New employees are required to serve an employment probationary period of 180 calendar
days. The probationary period begins on the initial date of employment. The purpose of this
period is to give you the opportunity to determine if you can function effectively in the JT3
work environment and to give management the opportunity to evaluate your compatibility with
work requirements, policies, practices and procedures.

The Company follows Federal and State laws, and strictly adheres to a policy of equal
opportunity for all individuals, based on merit. We will expect your performance and conduct
to also meet high standards and any serious deviation from these expectations could result in
denial of access to our various work locations and/or loss of employment.

MEDICAL CERTIFICATION
04/06/2017

I, Joel Duncan, hereby certify the following:

1. I acknowledge that I have been offered a work assignment that may be at a remote location.
I acknowledge that the essential functions of the job I will be required to perform on my work
assignment have been explained to me. I understand that the medical services available at any
given work area within a remote location may be limited and can vary from treatment on an
emergency basis by a medical technician to physician care, on a restricted basis, in a medical
clinic. I under-stand that some locations are not near a hospital and that in the event of a
serious injury or medical emergency, I may have to be evacuated by air and/or vehicle and
there would be considerable delay in obtaining health care and medical treatment.

2. I agree to accept this assignment, and do so knowingly and voluntarily assuming the risk of
working at locations where health care availability may be limited, as described in this
Certification. I hereby release JT3 LLC, the Customer, or any person in the ownership,
operation, and maintenance of the remote location work areas where I may be working, from
any and all liability, claims or damages arising out of or in any way caused by the fact that
medical and health care personnel and facilities may be extremely limited.

3. I hereby certify and represent that (check one):

_______ I am not aware that I have any medical or physical condition that would create a
direct threat to my health or safety, or the health or safety of others, if I accept this offer to
work under the conditions set forth in this Certification;

OR
_______ I do have a medical or physical condition that would create a direct threat to my
health or safety, or the health or safety of others if I accept this work assignment under the
conditions set forth in this Certification, but there are reasonable accommodations that would
eliminate the risk or reduce the risk to an acceptable level. I agree to fully cooperate with the
company in determining whether my medical or physical condition does in fact create a direct
threat to my health or safety, or the health or safety of others, and whether the risk can be
alleviated through a reasonable accommodation. I understand and agree that such cooperation
may include voluntarily undergoing a physical examination, if the company so requests. If
given such a physical examination, I understand that it will be at company expense, that the
results will be released to the company and to me, and that this information will be used for the
sole purpose of determining whether my condition does in fact create a direct threat to my
health or safety, or the health or safety of others, and to determine whether the risk can be
alleviated through a reasonable accommodation.

PRE-EMPLOYMENT DISCUSSIONS WITH JT3 LLC

AND POST-EMPLOYMENT RESTRICTIONS

04/06/2017 Joel Duncan

Congress has passed laws restricting contractors from discussing employment with certain
Government employees or advisors. These laws include severe penalties for both prospective
employees and JT3. Therefore, the following will insure that you are aware of and in
compliance with the following laws:

41 U.S.C. Section 423 - The Office of Federal Procurement Policy (OFPP) Act.

Federal Acquisition Regulations (FARs) 3.104-4 lists a number of restrictions and prohibitions
that apply to persons having involvement in Government procurements. Present or former
Government employees are not permitted to reveal proposal or source selection information
gained by virtue of their Government employment. No person is allowed to improperly obtain
proposal or source selection information. Agency officials that are personally and substantially
involved in a procurement should obtain the advice of an agency ethics official prior to
continuing employment discussions.

Even if you are not a current or former Government employee, restrictions may apply to you if
you are an adviser or a consultant to a federal agency or an employee of a Systems
Engineering or Technical Assistance (SETA) contractor, or similar contractor, with respect to
any phase of an agency procurement for which JT3 is a "competing contractor".

Please ensure you have read and are familiar with FAR Subpart 3. 104. Copies are available
here, or through the Office of General Counsel or Judge Advocate General (JAG) Office for
your agency.

18 U.S.C. Section 207 and 208.

18 USC 208 prohibits Government employees from taking official actions that affect their
personal financial interests. 18 USC 207 prohibits former Government employees from taking
certain actions on behalf of a private contractor, that relate to matters that were under his or her
responsibility while employed by the Government.

The application of the above laws and regulations may vary based on the facts and
circumstances applicable to each individual. If you are a current or former Government
employee, you are urged to consult with your assigned ethics advisor to determine the
application of the above rules to your personal situation.

CERTIFICATION:

I certify that I have read and am familiar with the above requirements. In addition, if I am a
current or former U.S. Government employee, I certify that I will inform JT3 of any post-
employment restrictions that may apply to me after leaving Government service.
