1.0 PURPOSE
To ensure that all MDR's supplied by Contractors and Vendors follow the same format
and that the types of documents supplied are consistent with the project requirements.

2.0 SCOPE

All CPMM Contractors and Direct Contracted Vendors, supplying manufactured

equipment to the project.

3.0 DEFINITIONS

CPM CITIC Pacific Mining

MOR Manufacturing Data Record (MOR). A document containing all
manufacturing data records as required by the ITP, as defined in 5.3.
ITP Inspection & Test Plan (ITP) is a document issued by the Vendors which defines
the Inspection Requirements, test reports and records to be issued.
ID Inspection Drawings (ID) are drawings that provide information such as
dimensions that QNQC personnel use to check against.
WM Weld maps (WM) are drawings or sketches that are used to indicate the
location and identification number of welds on each piece of equipment,
pressure vessel or piping.
QA/QC Quality Assurance/Quality Control

4.0 RESPONSIBILITIES

Project Managers The Project Managers are responsible to ensure that Vendors
follow the requirements of this procedure.
QA/QC Engineers Are responsible to ensure that the Vendors compile the MOR in
accordance with the ITP and that this is done on an ongoing
base.
Moody International When representing CPM Moody International (Ml) is the
preferred independent inspector responsible to ensure that the
Vendors compile the MOR in accordance with the ITP and that
this is done on an ongoing basis.

5.0 PROCESS

5.1 MANUFACTURING DATA RECORD

The Manufacturing Data Record (MOR) will be prepared as a stand-alone data book,
independent of the VENDOR Data Book. The MOR is the complete collection of all
certificates, test reports, inspection documents and any other document relevant to
manufacturing, inspection and testing activities performed during the supply, as per
the ITP, Purchase Order and relevant technical attachments.

The MOR index shall be submitted and approved by CPM prior to works
commencing. The MOR index shall have a different DI number from the MOR and
they shall de considered discrete contract deliverables.

The MOR shall be progressively compiled in line with the approved index and should
accurately reflect the stage or status of current works at all times. The MOR shall be
made available for review at all times at the contractor's place of construction or
alternatively the place of MOR compilation.

The completed (reviewed and approved) MDR(s) shall be submitted in electronic

format through CPM Coreworx database, and hard copy format.
The MOR contents shall be in accordance with the following:
All documents to be submitted in English or dual language
(English/Chinese).
Have a clear index and the sheets shall be properly numbered.

Be compiled on an ongoing basis by the VENDORS and kept available for

the Inspectors review at any time.
Material Certification Requirements shall be implemented in the requisition
and Data Sheet and defined by the Discipline Engineer. (Traceability is
required for all pressure containing or load bearing parts).
The Inspector shall give final approval of the MOR contents. At the final
test, the Inspector shall check the MOR for completeness and correctness.
The Inspector shall not release any equipment or material without MOR
final acceptance.
For materials not subject to an inspection and quality release, the relevant
MOR will be sent to the project office in Perth and will be available for
review if required.
For piping components (pipes, fittings, flanges), Material Certificates will be
submitted by the VENDORs, along with the "Material Certificate Summary"
completed.

Ensure traceability of material certificates for piping components during
construction, enabling access to the Construction team through Coreworx.
The MOR is considered to be a life time record and must therefore be
stored for the operational life of the project.

5.2 HARD COPY BINDER DETAILS

MOR Hard copy binders shall use four, 6mm round holes on an 80mm spacing and
with a 25mm margin.

Binders will be 3 " (70mm) spine with 4- hole posts in white PVC with transparent
PVC pocket on cover and spine for cardboard label inserts.

Note: SUPPLIER SHALL NOT SUPPLY THE MOR IN SPIRAL BINDERS

Binder Cover and Side Label shall be printed on cardboard and inserted in Binder's
front and side transparent pocket (See Front Label and Side Label master here
attached). Each label shall be filled in as per relevant explanations. (The required
numbering shall be in accordance with the project requirements)

5.4 ELECTRONIC COPY

The MOR Electronic Copy will be submitted to CPM via Coreworx by transmittal, only
after the Hard Copy has been approved by the CPM QA/QC Department.

Before the electronic version can be sent to CPM the MOR needs to be formatted
correctly as per the CPM Work Instruction Dl-170589 "Submission of Electronic
Manufacturing Data Records to CPM", the Work Instruction can be located in the
Supplier Area on Coreworx in "CP Mining/Supplier Area/Documents and Forms".
