LANDLORD APPROVAL FORM

DEAR PROSPECTIVE DIRECTV CUSTOMER,

Congratulations on your decision to sign up for DIRECTV service! You have made the best choice in digital television entertainment,
and we are confident you will be thrilled with the service. To ensure you have a smooth installation experience, if you do not own the
residence in which you intend to install your DIRECTV System, we ask that you obtain landlord approval prior to any installation.
Installation of a DIRECTV System may involve modifications to the residence. While these modifications are often minor, standard
professional installation many times includes drilling holes in order to run cable and attach equipment securely to the outside of
the structure. Any such type of modification may be forbidden pursuant to the terms of your lease/rental agreement or may require
pre-approval by the landlord. Please be advised that landlord approval is often required with multi-tenant apartment buildings.
You should carefully review your lease agreement to determine if you have the right to make such modifications. If landlord
approval is required, please request that the landlord, or its authorized representative, complete section 1 below to acknowledge
authorization for the installation and provide this signed form to the DIRECTV technician at the time of installation. In the alternative,
if the landlord verbally gives you permission or if landlord approval is not required pursuant to your lease or rental agreement,
please complete section 2 below.
By providing this signed form to your professional installer, you are granting DIRECTV permission to install your new DIRECTV
System and are releasing DIRECTV from obligation to remove your system after it is installed. The signed installation permission
form releases DIRECTV (including any installation technician) from any liability related to damages your landlord may claim as a
result of an alleged violation of your lease/rental agreement as it relates to the installation of your new DIRECTV System.
Should your landlord only approve the DIRECTV System installation subject to special requirements that make the actual install
more expensive (or exceeds what DIRECTV offers as part of its standard professional installation), you will be asked to pay a
corresponding charge, to be agreed upon by you and your installer, for the additional services. In addition, please remember that
technical issues may still prevent installation of your DIRECTV System. Though this is rare, challenges such as a poor line-of-sight
to the satellite, or inadequate grounding availability, can prevent installation of the DIRECTV System, in which case your money

digital y estamos seguros de que disfrutars en grande de nuestro servicio. Para asegurar que tengas una buena experiencia
durante la instalacin, si no eres dueo/a de la vivienda en la que instalaremos el sistema de DIRECTV, te pedimos que obtengas
la aprobacin del propietario antes de la instalacin.
La instalacin del sistema de DIRECTV podra incluir modificaciones a tu vivienda. Aunque estos cambios usualmente no son
cambios mayores, la instalacin profesional estndar muchas veces requiere que el tcnico haga agujeros con un taladro para
pasar el cable y sujetar bien el equipo afuera de la estructura. Este tipo de modificaciones podran estar prohibidos con base en
los trminos de tu contrato de alquiler o podran requerir de la aprobacin previa del propietario. Por favor toma en cuenta que la
aprobacin del propietario usualmente es un requisito en edificios de apartamentos con mltiples inquilinos.
Te aconsejamos que consultes cuidadosamente tu contrato de alquiler para determinar si tienes derecho a hacer estos cambios.
Si se requiere de la aprobacin del propietario, por favor pdele a ste, o a su representante autorizado, que complete la seccin
#1 en este formulario diciendo que autoriza la instalacin. Cuando el tcnico de DIRECTV vaya a hacer la instalacin, por favor
proporcinale este documento firmado por el propietario o representante autorizado. Como alternativa, si el propietario te da la
autorizacin verbalmente o si no necesitas su aprobacin con base en los trminos de tu contrato de alquiler, por favor completa
la seccin #2.
Al proporcionarle este documento firmado al instalador profesional, ests dndole permiso a DIRECTV para que instale tu nuevo
sistema de DIRECTV. Adems, ests liberando a DIRECTV de cualquier obligacin de remover el sistema despus de haber sido
instalado. El permiso de instalacin firmado tambin libera a DIRECTV (y a cualquier tcnico que haga la instalacin) de cualquier
responsabilidad relacionada a los daos que el propietario de tu vivienda pudiera reclamar como resultado de una violacin a tu
contrato de alquiler en relacin a la instalacin de tu nuevo sistema de DIRECTV.
Si el propietario de tu vivienda decide aprobar solamente la instalacin del sistema de DIRECTV bajo ciertas condiciones especiales
que hagan que la instalacin sea ms cara (o superior a lo que DIRECTV ofrece como parte de su instalacin profesional estndar),
debers pagar el cargo correspondiente, el cual ser acordado entre t y el instalador, para cubrir los servicios adicionales.
Adems, por favor recuerda que ciertos problemas tcnicos podran prevenir que el instalador complete la instalacin de tu
sistema de DIRECTV. A pesar de que estas situaciones no suceden comnmente, ciertos desafos como un campo visual reducido
para la antena satelital o disponibilidad inadecuada para conectarlo a tierra, pueden prevenir la instalacin del sistema de
DIRECTV. En tal caso, te devolveremos tu dinero.
